| Objective:This paper mainly introduces the dialectical thinking of MD. Liu Ruxiu and the unique clinical diagnosis and treatment experience for premature ventricular contractions patients. And also summarizes her original characteristic using of Chinese herbal medicine. The kidney-and-heart-nourishing formula as my mentor s the most oftenly used prescription for palpitations, it contains precious TCM treating concept which she inherited off her father Liu Zhiming, one ofthe old experts and masters of Chinese medicine. It aggluts the Ireasurous wisdom of two generations physicians accumulated over decades of clinical hard work, and has greart value that worth a further in-depth discussion. By observing and studying the patient’s relief degree and objective examination improvements before and after taking the formula, we can give a preliminary evaluation of the efficacy of kidney-and-heart-nourishing formula. Meanwhile this clinical observation lays the foundation for a further investigation of this formula’s pharmacological mechanism.Method:I select 30 cases of premature ventricular contraction that MD.Liu has treated from cardiovascular department of the Guanganmen Hospital of Chinese Academy of traditional Chinese Medicine strictly according to the provisions ofthe inclusion criteria. Before the clinical observation begins i collect their basic information and medical history data and fill in the curative effect observation form. After a 4we:ek-bng term of kidney-and-heart-nourishing formula treatment, I compare with the before and after symptoms’changes, BCGs,and 24-hour Holler monitoring results. Based on this evidences I can give a preliminary evaluation of clinical curative effect ofthe formula, whilst provide a proof ofthe effectiveness of traditional Chinese medicine treatment and the academic method of MD.Liu RuxiaResult:The observation consists of 27 cases in total,13 cases of male, female 14 cases. After traditional Chinese medicine treatment, the patients’ palpitation and other following symptoms of palpitation were significantly improved, such as chest tightness, fatigue, shortness of breath, and etc. cure rate at 11.11%, excellence rate at 40.74%, effective rate at 48.14%; Compared 24 hour ambulatory ECG data of before and after treatment shows the number as well as the ratio of premature ventricular beats decreased significantly,10 cases were markedly effective,10 cases were effective, and 7 cases were ineffective,74.07% was the total effective rate; after the treatment, Myerburg grade total score was reduced,17 cases were markedly effective,4 cases were effective, the total effective rate was 77.78%,6 cases were ineffective, as 22.22% of all caese; heart rate variability analysis results showes that useage of this formula can increase pNN50, rMSSD, SDNN, the effective rate was 62.96%, and the effective rate was 14.81%. After the calculation the above indicators’ P<0.01, and of statistical differences. Comparing the changes of safety representative index such as HGB, ALT, Cr, BUN before and after treatment, p>0.05, means no statistical differences.Conclusion:In palpitation patients with heart and kidney incoordination, Kidney-and-heart-nourishing formula has relieved various showed clinical symptoms and discomfort, reduct the ventricular premature beats in number and and the onset times, helped to reduce the number of ectopic pacemaker, Has reached the clinical effect of reducing the patients’score in Myerburg classification, suggested that the larget of this prescription may be the autonomic nervous system of the heart by tracing title change of heart rate variability index. In addition the formula with no adverse effects on the patients’ blood routine, liver and kidney function. |
