Two parts are included in the dissertation, literature review and clinical research.The first part is the literature review, mainly introduces the background of the research, which is the progress of research on the relationship between malignant tumor and diabetes mellitus, including the following sections:1. Research status of western medicine for malignant tumor complicated with diabetes, contains the basic concept and epidemiology about malignant tumor and diabetes, pathogenesis of diabetic secondary malignancies.2.The etiology and pathogenesis of malignant tumor and diabetes mellitus in Chinese medicine, and the understanding of malignant tumor complicated with diabetes in Chinese medicine. The second part is a clinical study entitled Study on the effect of chemotherapy on fasting blood glucose and TCM syndrome in patients with lung cancerObjective:To investigate the study mainly from three aspects, (1) the effect of chemotherapy on fasting glucose; (2) the effect of chemotherapy on TCM syndrome; (3)The relationship of the glucose and TCM syndromes.Methods:A retrospective study of statistical analysis were used, collecting 122 medical records of patients with lung cancer in China-Japan Friendship Hospital Integrative oncology from 1 March 2015 to 29 February 2016. Research analyzing factors contain the patients’age, gender、pathologic type、times of chemotherapy、chemotherapy regimen、tumor stage、 the use of hormone before the chemotherapy, fasting blood glucose level before and after chemotherapy, some patients’glycated hemoglobin, TCM syndrome。By analyzing these factors, we can explore the influence of chemotherapy on fasting glucose and TCM type.Results:1. fasting blood glucose levels after chemotherapy were significantly higher than the average level before chemotherapy, comparing the two groups have significant difference (P<0.01) 2.Sex, age, tumor stage, times of chemotherapy, pathological types have no significant differences in effecting fasting blood glucose levels.3. Hormone’s effect should analyze in detail, the result between large-doses group and low-dose group has significant difference (P<0.05), but the contrast of low-dose hormone group and non-hormone group has no statistical significance (P>0.05); In contrast to 4 different chemotherapy regimens, the average blood glucose level of docetaxel group and paclitaxel group are located in the first and second place, the two group have a higher blood glucose level compared with etoposide group, pemetrexed group (P<0.05), the comparision between other groups is not statistically significant (P>0.05);5.Before the chemotherapy phlegm-damp, stagnation of phlegm-heat in lung, blood stasis, syndrome of qi stagnation and blood stasis phlegm are the main TCM syndrome; after chemotherapy, qi and blood deficiency, qi Yin Deficiency are the main syndrome; 6. After chemotherapy, taxane group based on the type of Yin qi deficiency based; the gemcitabine group, etoposide group, pemetrexed group, Irinotecan group after chemotherapy is based on both qi and blood deficiency、phlegm-damp syndrome; 7.Before chemotherapy, Abnormal blood glucose group’s types of syndrome are qi and blood deficiency,phlegm-damp syndrome,blood stasis. After chemotherapy, the main TCM syndrome are qi yin deficiency and yin deficienc,y.Conclusions:1.Chemotherapy has a significant effect on fasting blood glucose, chemotherapy can lead to hyperglycemia, and even secondary diabetes; 2.The factor of age, gender, times of chemotherapy, tumor stage had no effect on fasting blood glucose; 3.Small doses of hormone have no significant effect on fasting glucose, a larger dose may increase the glucose level; 4. Taxane chemotherapy can increase the fasting blood glucose, it has the greatest impact on blood glucose; 5.Chemotherapy side effect on blood glucose may be temporary; 6.Before chemotherapy, the patients with high blood glucose level may be a risk factor for hyperglycemia after chemotherapy; 7.The TCM syndrome are mainly phlegm-damp syndrome,blood stasis and qi stagnation before chemotherapy; After chemotherapy,the TCM syndrome are mainly deficiency of qi and blood, yin deficiency, Qi and Yin deficiency, which means chemotherapy drug can injure blood Yin fluid; 8. After different chemotherapy, the change of the TCM syndrome is not the same; 9.After chemotherapy, the patients with abnormal glucose metabolism mainly have deficiency of yin and Qi,yin deficiency.
