Background:Chronic eczema is a common clinical disease in the Department of dermatology. From the eczema itself, it has the characteristics of easy to chronic development, easy to repeated attacks, with itching as the most prominent symptoms of consciousness. Hand and foot chronic eczema is due to the exposed parts, it is difficult to ignore and embarrass, and itching caused by scratching, brings to the patients with persistent physical and psychological two-sided, also makes the patient desire strong to treat. Therefore, the study of the treatment of hand and foot eczema, has a good clinical significance. Modern medical treatment of eczema, there are still many deficiencies, such as easy to relapse after treatment, individual resistance, etc. In conclusion, clinical practice in the principle of existing treatment under the guidance of, still need to broaden their horizons, found and value the Chinese medicine in the treatment.Objective:This study through clinical trials, observe the curative effect of our homemade No.2 hydropathic compress prescription of hand and foot chronic eczema treatment, and security. Further summarizes, optimize and improve the prescription, hope can provide TCM treatment of chronic eczema with some new ideas and methods.Research methods:the clinical observation test of all the research object, from November 2015 to February 2016 period, China academy of traditional Chinese medicine doctor gate hospital dermatology clinic patients, all included in the study were diagnosed by clinical physicians, conforms to the diagnostic criteria of chronic eczema, and skin lesions limited on the hands or feet. A total of 63 cases of chronic eczema were collected. Random grouping of the envelope method with the time of entry. Patients in the treatment group were given wet compress with homemade No.2 hydropathic compress prescription, daily morning and evening once treatment, cold on the surface of the skin in the affected area; control group gave tretinoin cream, external use 1 times each morning and evening, and pay attention to sunscreen or other means of UV exposure. During the test for each patient established clinical case report form, pruritus, erythema level before treatment and weekly visits, infiltration degree and degree of squamous scratches, infiltration hypertrophy moss degree five symptoms and target lesion area of scoring records. The course lasted for 4 weeks. Research results:3 cases of loss is not included in the data statistics. Treatment group,30 patients recovered in 1 case,7 cases were markedly effective, effective 20 cases, ineffective in 2 cases, the markedly effective rate was 26.67%, total effective rate was 93.33%; The control group of 30 patients,1 case was cured,1 case had marked effect,17 cases effective, ineffective in 11 cases, markedly effective rate was 6.67%, total effective rate was 63.33%. Chi-square test results P values were 0.038 and 0.005 (< 0.05), with significant difference. Itch treatment, treatment group healed in 2 cases,12 cases had marked effect,10 cases,6 cases ineffective, the markedly effective rate was 46.7%, the total effective rate was 80%; Control group healed in 2 cases,3 cases were markedly effective, effective 15 cases,10 cases of invalid, markedly effective rate of 16.7%, the total effective rate was 60%. Itchy statistical analysis, two groups of curative effect comparison, markedly effective rate has a significant difference (P= 0.012, P< 0.05); The total effective rate difference has no obvious statistical significance. Lesion area of comparative effectiveness, the treatment group recovered in 1 case,3 cases were markedly effective, effective in 5 cases,21 had no effect, the markedly effective rate was 13.33%, total effective rate was 30%; The control group recovered in 1 case, 0 cases had marked effect,12 cases effective,17 had no effect, the markedly effective rate was 3.33%, total effective rate was 43.33%. Statistical analysis, two groups of area of effect of contrast, there was no obvious statistical significance difference. Treatment group and control group in comparison before and after treatment, her lesions score, modified EASI scores, degree of pruritus score all have different degree of decline, the statistical test, think differences are significant (P< 0.05). Two groups improved EASI scores, degree of pruritus score decreased degree of comparison, the treatment group were superior to control group, after statistics test, no significant statistical difference pruritus score loss, improve EASI scores have significant difference (P< 0.05). Skin lesion, two groups of erythema, papules, scales, infiltrating comparison score loss, erythema, scales scratch score a degree of decline, the treatment group were superior to control group, the statistical test, with significant difference (P< 0.05), P= 0.004 including erythema score loss, P< 0.01; Papule grade, infiltration hypertrophy moss, control group is better than that of the treatment group, after statistics test, no significant statistical difference papule score loss, infiltration hypertrophy moss change scores decline degree has significant difference (P< 0.05)Conclusion:the therapeutic effects of homemade No.2 hydropathic compress prescription of hand and foot chronic eczema is obvious, especially for pruritus and erythema, scaly skin effect is good, but for the improvement of the lesion area of effect is not obvious, may be associated with drug use time is shorter. This study has certain reference value for the clinical treatment of chronic eczema, for a better treatment of chronic eczema provides experience and thought, has the value of further research.
