Objective: Eczema is a common skin disease in clinic, and can be classified into three stages which include acute stage, subacute stage and chronic stage. In the acute stage, papule, blister, scab and erythema are clinical features; but in the chronic stage, dryness, scales and lichenification are clinical symptoms; and pruritus is a concomitant symptom in every stage. Chronic hand and foot eczema is a special kind of eczema, and dryness, pachylosis, keratinization and rahagades are clinical characteristics with obvious pruritus and long duration. According to clinical experience, chronic hand and foot eczema is common seen in middle aged and elderly people. Considering this disease is relative to the changes of sex hormones, this research observes the clinical efficacy of only using Chinese herbs based on the principles of tonifying kidney and spleen to treat chronic hand and foot eczema.Methods:All patients in this study came from the patients who treated in Dermatology clinic of Guang’anmen Hospital of CACMS from May2012to September2012. Chronic hand and foot eczema happened in women aging45-55years old and men aging50-60years old are satisfying criteria and total38cases were selected and completed. Therapeutic method is only using Chinese herbs and the prescription is tufuling, shanyao, zhishouwu, dangshen and sticky rice, twice a day and three weeks. In the study process, each patient had own clinical observation table, and erythema, keratinization, rahagades, area and pruritus were used as the observation targets with the four-level scoring standard.Results:After three weeks of treatment and in the total38cases,7cases were markedly effective,18cases were effective and13were ineffective, the total effective rate was65.79%, and the result of Wilcoxon test was P<0.05which indicated this treatment was effective. On the other side, the results of each scoring standards were:I Condition of erythema: pre-treatment score0was1case, score1was4cases, score2was24cases and score3was9cases, and after treatment score0was10cases, score1was16cases, score2was11cases and score3was1case; II Condition of keratinization:pre-treatment score0was0case, score1was1case, score2was17cases and score3was20cases, and after treatment score0was2cases, score1was25cases, score2was11cases and score3was0case;Ⅲ Condition of rahagades:pre-treatment score0was0case, score1was19cases, score2was15cases and score3was4cases, and after treatment score0was17cases, score1was18cases, score2was3cases and score3was0case; IV Area of lesion:pre-treatment score0was0case, score1was10cases, score2was15cases and score3was13cases, and after treatment score0was0case, score1was19cases, score2was6cases and score3was13cases; V Condition of pruritus:pre-treatment score0was1case, score1was3cases, score2was16cases and score3was18cases, and after treatment score0was6cases, score1was14cases, score2was14cases and score3was4cases; and the results of Wilcoxon test of each scoring standard were P<0.05which indicated five aspects of lesion condition were improved treated by this therapy.Conclusion:Chinese herbs (tufuling, shanyao, zhishouwu, dangshen and sticky rice) based on the principle of tonifying kidney and spleen had definite curative effect on treating chronic hand and foot eczema happened in climacterium. Five aspects of lesion condition, which were erythema, keratinization, rahagades, area and pruritus, were all improved in a certain extent, and the results of this clinical research indicated this method of treatment brought up a new thought of therapies in treating chronic eczema.
