Background:Acute myocardial infarction (AMI) is kind of disease of acute coronary syndrome (ACS), which are more aggressive, poor prognosis and even seriously threated to human health. Whether it can be early diagnosed and promptly treatment plays a key role in increasing survival rate, and improving the prognosis and life quality in patients with AMI. Myocardium markers are important for early dignosis of AMI, including troponin (cardiac troponin, cTn), creatine kinase (isoenzyme MB of creatine kinase, CK- MB) and myoglobin (myoglobin, MYO). cTn and CK-MB appears late though they are high specificity. MYO appears early but is lack of cardiac specificity. Therefore, it is necessary to find certain myocardium markers for early diagnosis of AMI. In recent years, heart fatty acid binding protein (heart-type fatty acid-binding protein, H-FABP) and ischemia modified protein (ischemia modified albumin, IMA) are attracting widespread interest.Objective:To investigate the value of five myocardium markers (cTnT, CK-MB, MYO, H-FABP and IMA) with quantitative analysis on early diagnosis of acute myocardial infarction (AMI).Methods:A total of 20 patients with acute myocardial infarction (AMI) were enrolled as experiment group, while 20 health volunteers served as control. Concentration of five myocardium markers cTnT, CK-MB, MYO, H-FABP and IMA were measured in two groups. Statistic analysis was respectively performed T-test and rank sum test by using SPSS 10.0 statistic software. The diagnostic results were evaluated with ROC curve analysis.Results:There is significance difference of five myocardium markers between two groups (P<0.05). cTnT with ROC curve can not be done in control group for its concentration was lower than 0.01 ng/ml. The area under ROC curve of H-FABP was significantly larger than that of CK-MB, IMA and MYO. The results showed that H-FABP was much valuable for diagnosis of AMI.Conclusion:H-FABP has some advantage in early detection of AMI compared with CK-MB,IMA,MYO, can be regarded as one of perfect markers for early clinical diagnosis of AMI,more suitable for clincal application.
