| IntroductionThe domestic health service has been developed rapidly and the quantity and quality of health resources has been greatly improved since 1978. However, due to lacking of the macro-control, allocation of health resources fragmented, irrational structures, poor efficiency is more and more prominent as the general state of our country. Both waste and shortage of resoures exists simultaneously.The objects of this study are to investigate the status of health organizations in Zhengzhou City, to find out the problem, to promote the rational allocation and the efficiency of the utilization of health resources, to provide necessary reference for the formulation and implementation of the regional health plan in Zhengzhou City.Material and Methods1. Having 267 medical institutions above township hospital as research subject in Zhengzhou City. To census these 267 medical institutions and all of their employees, including the number and distribution of medical institutions, health resources in human, fixed assets, workload and working efficiency.2. The investigation forms were designed previously, including the investigation forms of medical institutions and the investigation forms of medical and health personnel .The medical resources were all investigated in the 267 medical institutions. 3. The data base was set up using the software of Epidata3.1. Date processing and statistic analysis was carried out by using the softwares of SPSS10.0 and EXCEL2003.Results1. The general situationThere were 267 medical institutions(excluding the ones lower than township hospitals) in Zhengzhou City. By agency type of classification, 113 comprehensive hospitals shared of 42.32 percent and 101 township hospitals shared of 37.83 percent are higher, while specialized hospitals and hospitals in traditional Chinese medicine were less; according to geographical classification, 126 medical institutions were in Zhengzhou downtown area,accounting for nearly half of the number of medical institutions in Zhengzhou City, while other cities and counties were smaller more or less. The average level of number bed per thousand population was 4.14 in Zhengzhou City, significantly higher than the national average of 2.42.2. Health resources situation in human2.1 There were 43236 health workers in Zhengzhou City, of them, 35039 health professionals, accounting for 81.12%; 3865 ground crew workers, accounting for 8.94%; 1388 administrative staff, accounting for 3.21% . 25519 health professionals were in Zhengzhou downtown area, accounting for 72.83% of health technicians in Zhengzhou City; 24643 health professionals were in comprehensive hospitals, accounting for 70.33% of health professionals in medical institutions.2.2 In Zhengzhou City, clinical practitioners accounted for 25.93%, physicians assistants accounted for 3.91%, practicing nurses accounted for 30.61%, Pharmacists accounted for 2.61%, and others accounted for 11.46% and non-qualifications accounted for 25.48%.2.3 The health workers in Zhengzhou, the senior professionals accounted for 8.98%; intermediate professionals accounted for 24.69%; junior accounted for 42.12%; others accounted for 24.21%. In township hospitals the senior professional titles accounted for 0.51%, intermediate professional titles accounted for 8.83%, far less than other medical institutions; In Zhengzhou downtown area, senior professionals accounted for 10.96% ,which was far higher than other cities and counties, intermediate professional s accounting for 26.87% were slightly higher than other cities and counties.2.4 Education level of health workers in Zhengzhou City, doctors accounted for 0.73%, masters accounted for 2.96%, undergraduates accounted for 20.61% and associate degrees accounted for 62.72%, non-professionals accounted for 9.95%. The proportion of which undergraduate Master and Ph.D was in Zhengzhou downtown area and general hospitals was significantly higherr than that in township hospitals.3. Analysis of fixed assets utilization3.1 There was a total value of 4.72527 billion yuan in fixed assets of the medical institutions in Zhengzhou City, Zhengzhou downtown area had 3.91683 billion yuan, accounting for 82.9% of the total, significantly higher than other cities and counties, while other cities and counties all had only about 3% of the total; comprehensive hospitals accounted of the total 66.95%, significantly higher than other medical institutions . The lowest proportion of township hospitals was only 3.2%. The volume of fixed assets in township hospitals was far insufficient.3.2 The ratio of beds to doctors in Zhengzhou City was 1.99. The difference between various cites and between different medical institutions was not significant. The ratio of doctors to nurses in Zhengzhou City was 0.91, the lowest was 0.85 in Zhengzhou down town area, as followed by Dengfeng City. The other regions were slightly higher than 1. The ratio of beds to doctors in the various medical institutions were about 0.91. The highest was 1.83 in township hospitals while the lowest was 0.79 in the comprehensive hospitals.4. Analysis of the workload and working efficiencyIn 2006 in Zhengzhou City, the workload of health personnel per capita outpatient visits was 1062, per capita emergency was 84.87. The average annual hospitalization number was 46.5. Township hospitals per capita annual outpatient workload was 1434.96 and emergency room visits per year was 221.01.The difference between medical institutions in average annual per capita hospital was not significant. Conclusions1. There were more medical institutions in Zhengzhou downtown area than in other cities and counties, large general hospitals and township hospitals.The average number of beds per thousand population in Zhengzhou City was significantly higher than the national average level.2. There were 43236 health personnels in medical institutions above in Zhengzhou City. The number of health professionals per thousand population, the number of doctors per thousand population and the number of nurses per thousand population in Zhengzhou City were more than other city and county. There were more health personnels with senior title in Zhengzhou downtown area than that of medical institutions in other cities and counties. The proportion of Ph.D and master was small and generally low academic titles and education in the township hospitals.3. The number of the total fixed assets and the beds in Zhengzhou downtown area and general hopitals are much larger than in other cities and counties and other medical institutions, the number of beds and health care bed ratio in some medical institutions are irrational.4. The number of out-patient, emergency care, hospital admissions in Zhengzhou urban area and the general hospital are larger than any other city and county and medical institutions, while the highest per-capita volume of outpatient and emergency department, most efficient in the township hospitals. |
