Objective To investigate the possible pathogenetic role of P16 and P15 gene expression in the genesis and development of human gestational trophoblastic disease, including non-invasive hydatidiform mole , invasive hydatidiform mole and choriocarcinorma Material and methods Tissue sample taken from 21 cases of invasive hydatidiform mole and choriocarcinorma , 20 cases of non-invasive hydatidiform mole and 20 cases of normal villi were studied by immunohistochemistry using two monoclonal rabbit antibodies that react against P16 and P15 gene products (P16 and P15 protein). Result A low positive expression of P16 and P15 in malignant gestational trophoblastic neoplasrns(33.3% and 40.9% )was observed, with significant difference from normal villi(75. 0% and 80. 0%) (P<0. 01, P<0. 05). There was difference between P16 and P15 positive expression in choriocarcinorma (22. 2% and 33. 3%) and in non-invasive hydatidiform mole(55.0% and 60,0%) (P<0.05).The two genes positive expression showed opposite relationship with grading, FIGO stage, duration of hCG in blood or urine and prognosis (P<0. 05). P16 and P15 gene co-negative expression was observed in 50. 0% hydatidiform mole cases which had become invasive hydatidiform mole and choriocarcinorma and 15. 0% other hydatidiform mole cases. The expression of P16 in different groups was more notable than that of P15. Conclution Abnormal expession of P16 and P15 gene plays an important role in the genesis and development of trophoblastic neoplasm, and is related to the grading, FIGO stage and prognosis. P16 and P15 proteins may be a useful predictive indicator for carcinomatous change and could guide clinical treatment.Also,they could provid the theoretical foundation and experimental evidence of further gene therapy.
