Objective For explore the pathogenesis of bronchial asthma and seek the effective method to asthma, we investigated the expression of IL-8,IL-10 and IFN-γ in serum and PBMC from bronchial asthma patients, as well as observed the change of the cytokines and clinical effect after administration with two treatments.Method According to the criterions of diagnosis and degree established by Respiratoy Branch of Chinese Medicine Academy, 81 patients of mild or moderate bronchial asthma was selected . We administer twe therapies with Theophylline Sustained Release Tablets (PROTHEO(r)) + Beclomethasone Dipropionate inhaler (Becotide 50 inhaler) or Beclomethasone Dipropionate inhaler (Becotide 50 inhaler) . Blood serum and PBMC smear was taken before and after treatment in asthma patients. The level in serum or mRNA expression in PBMC of IL-8,IL-10,IFN-γ were determined with ELISA and in situ hybridization. Clincal curative effect was observed simultaneity. All data were treated with SPSS 8.0 statistic software. Results The level and expression of IL-8 in attack period of mild or moderate bronchial asthma patients increased significantly (p﹤0.01), but IL-10,IFN-γ decreased significantly (p﹤0.01) compared to normal control groups. There were significant change in cytokines expression in serum and PBMC after treatments(IL-8 down-regulation,p﹤0.01;IL-10,IFN-γ up-regulation,p﹤0.05) . There were no significant change in cytokines expression and clinical curative effect after treatments A project compared to B project(p﹥0.05). There was no concentration dependence relationships between theophylline concentration and those cytokines level(p﹥0.05) , theophylline concentration in A project group was 4.96±1.80μg/ml.Conclution the level of IL-8 in attack period of mild or moderate bronchial asthma patients increased significantly, but IL-10,IFN-γ decreased significantly compared to normal control groups show three cytokines participating in the regulation of bronchial asthma pathogenesis. There was satisfactory clinical curative effect in asthma patients by two treatments, no significance of difference . There were significant change in cytokines expression in serum and PBMC after two treatments(IL-8 down-regulation,IL-10,IFN-γ up-regulation), infered that it?s one of the mechanism of anti-inflammation of glucocorticoid. There was no concentration dependence relationships between theophylline concentration and three cytokines level, therfore we presume that low theophylline concentration could bring into play both of anti-inflammation and control wheezing. Without significant difference , it affect on those cytokines in two therapies (p﹥0.05). Therefore,we deduce that Theophylline combine with Glucocorticoid could have synergetic effect ,but without adding effect.
