ObjectionAs to treatment of acute focal cerebral ischemic, thrombolysis is publicly taken as one of effective therapies, which has been increasingly regarded, however, reperfusion injury followed it confines the apply clinically. Many recent studies indicate ischemic-reperfusion damage is a multi- step pathologic and physiological process. Chinese traditional medicine compound has various chemical component and broad biology activity, which provides the possibility on theory and practice for holding up the multi-approach and multi-level injury from ischemic-reperfusion damage. The study investigates the protective effect and explore the protective mechanism of BHID(Buyanghuanwu improvement decoction) that had been identified effective clinically, which can provide scientific proof to enlarge the clinical apply of the same kind Chinese traditional medicine and the new preparation of cerebral protection. MethodsNinety-six of Wistar rats (Provided by Medical College in Wuhan University)were randomly divided into three groups: sham-operated group, control group and treatment group. The focal cerebral ischemic-reperfusion model was established with thread embolism of middle cerebral artery. After suffering 2 hours occlusion and following 24h and 72h reperfusion separately, we observed the protective effect of BHID on the change of the whole situation, infarct volume, cerebral water content, damage degree of brain function, the pathology of brain tissue with light microscope and electron microscope., apoptosis with Flow Cytometry, the expression of bFGF(basic fibrosis growth factor)with immunohistochemitry techniques. Resultsl.BHID can significantly improve the whole situation and brain function after focal cerebral ischemic reperfusion. After operation, the psychosis and hair colour of the rats in sham-operated group is good. Compared with the sham-operated, the control group and treatment group were more poor in energy, dimmer in hair, reductive in spontaneous activities and bite and sup, degreessive in the sensitivity on environment, the situation above of treatment group were gradually improved, but control group not . The neurologic function score in the treatment group was lower than in the control group(P<0.01).2.BHID can reduce the cerebral edema and cut down the infarct volume after focal ischemic-reperfusion in rats. The infarct volume and the cerebral water content intreatment group were less than in the control group. There was a thoroughly distinct difference between the two groups above(P<0.05 or P<0.01)3.In the infarcted mature area of the control group, the neuron , gliocyte and vessel were all necrotic, and cytoskeleton disappeared. Beside the infarcred mature area, there were large number of dark neurons, shrinking cekks with destroyed, cytoskeleton, active microglia and inflammatory cells. Compared with the control group, the pathologic injury degree of cerebral tissue in the treatment group was significant improved under light microscopy and electron microscope.4.The incidence of neuronal apoptosis in the control group and the treatment group was more distinctly increasing than that in the sham-operated group. Compared with the control group, the incidence of neuronal apoptosis of the treatment group was less decreasing .There were a quietly marked difference between at 24h and at 72h of the treatment group(P<0.01).5.There were few expression of bFGF at the parasagittal cerebral cotex of sham-operated group. The expression in the control group was enhanced, and the expression at 24h was more than that at 72h(P<0.05). Compared with the control group ,the expression of bFGF of the treatment group was higher. There was a more marked difference at various time of the expression of bFGF between the treatment group and the control group. (P<0.01), and at 72h the treatment group still kept high expression. Compared with 24h, there was no obvious difference.(P>0.05). ConclusionBy the mechanism of boosting the expression of bFGF and anti- apoptosis,the decoction can lighten cerebral ischemic-reperfusion damage...
