Color Doppler Ultrasound Measurements Of Cerebral Placental Blood Flow Than The Preliminary Study Of The Umbilical Cord Around The Neck Of Fetal Prognosis Evaluation

objective: we use color Doppler ultrasound to evaluate umbilical cord aroundneck fetus′s cerebroplacental ratio to observe if it is variety with gastationincreasing and evaluate its value in predicting prognosis in umbilical aroundneck fetus. Methods: there are 37 cases in umbilical around neck(UCAN)group and 43 cases in control group. we examine systolic/diastolic ratio(S/D),pulsatility index(PI) value of the umbilical artery and middle cerebral artery ofevery observed object.and calculate the CPR value which equal toPIMCA/PIUA.we had also divided these cases into 28～37⁺⁶ weeks group and 37～41⁺⁶ weeks group separately.Even contrast the final antepartum examinationresult (the time between labor and ultrasound examine is not exceed two weeks)with the postpartum follow-up study in the mature pregnant. Result: the CPRvalue is changing with gestational age and seems to a decline trend in ourstudy.there is no diffrence between the UCAN and control group(P>0.05); inthe 43 mature pregnant cases there are 36 cases in our follow-up study and 17cases had approved existing umbilical around neck one loop in the delivery.in the17 cases 15 cases apgar score≥7(14 cases CPR≥1,1 case CPR<1); 2 cases theamniotic fluid Ⅲ°meconium-stained (all cases CPR<1).we find these 2 casesreally exist hypoxia at diffrent degree from our follow-up study while excludethe placental, intrapartum time and other umbilical cord abnormity. In our study wecompare the postpartum follow-up result of the two CPR value via statistics.andCPR<1 than CPR≥1 has statistic sense (p<0.05).Conclusion: the DopplerCPR value can evalue fetus cerebral oxygen supply state,especially with tightenUCAN fetus and more sensitively when CPR <1.the CPR value can alsosupervise obstetric clinic to carry obstetric step to improve perinatal fetussurvive quality.
