The chronic renal failure & secondary hyperparathyroidism (CRF-SHPT) is a serious disease which do harm to the people's health. The calcium-sensing receptor (CaSR) and vitamin D receptor (VDR) can regulate the concentration of the serum calcium and the serum intact parathyroid homone. It is suggest that the CRF-SHPT may be influenced by the polymorphism of the CaSR gene and the VDR gene in the recent studies, but there are contrary viewpoints.In order to evaluate the effect of the CaSR gene polymorphism and the VDR gene polymorphism on renal hyperparathyroidism (CRF-SHPT) 122 CRF-SHPT Chinese patients and 25 normal persons from Jiangsu province was investigated. The intron5 polymorphism of CaSR gene and BsmI polymorphism of VDR gene was analyzed by the method of PCR-RFLP. The serum levels of calcium, phosphorus and intact PTH (iPTH) were assayed by standard methods. All the data were analyzed by statistical method.We find the results listed below in the study of CaSR gene polymorphisms. There is no significant difference between the CRF-SHPT patients and normal persons in genotypic frequency of intron5. It is suggested that the CaSR gene polymorphisms would not cause CRF-SHPT. The genotypic frequency of intron5 in the Han nationality is significantly different from the frequency in other nationalities. Different from previous researches, ethnic differences exist in the intron5 polymorphism. The TT patients show significantly lower serum levels of intact PTH and Ca than the CC group. It demonstrates that the CaSR polymorphisms of intron 5 were closely associated with the serum levels of PTH and calcium as well as the clinical severity in CRF-SHPT patients.The BB genotype was not found in the research of VDR gene. The investigation confirms that the BsmI polymorphisms have strong bias in Chinese. The Patients with bb genotype show lower serum levels of calcium and higher serum levels ofphosphorus than the Bb group. It seems that Patients with bb genotypes were more likely to develop to severe SHPT. Besides, in the patients with bb genotypes, the female is older than the male, and the female's condition is probably more serious than the male based on the physiological index calcium and phosphorus.It seems that the CaSR gene polymorphisms of intron 5 is associated with the VDR gene polymorphisms of Bsml. It was identified that the serum iPTH and Ca2+ level of BbCC genotype was higher than the other genotypes. The serum P level of BbCC was the lowest all of the genotypes. All the data suggest that the TT genotype patients together with bb genotype have more serious CRF-SHPT.
