ObjectiveTo discuss the TCM regulation of syndrome differentiation including distribution, variation and composement of psoriasis.Establish the standard of psoriasis principal syndrome differentiation.Through the clinical research of prescription-syndrome-effect, verify the accurate distiinguision and effective treatment of three famous TCM doctors' experience. MethodsThe research is based on the syndrome differentiation and typing of three famous TCM doctors including Zhu renkang,Zhao bingnan and Jin qifeng.Abiding by the principle of EBM,using the methods of clinical epidemiology and operating strict design,investigate the psoriatic TCM regulation of syndrome distribution and variation of psoriasis in the different sex,age,period,season and progress.Through the statistical analysis of body's symptom,tongue manifestation, pulse manifestation and lesion,discuss the regulation of psoriatic syndrome composement.Using the clinical research method of random,single-blind and contral,observe the curative effect about the syndrome of blood heat,blood dryness and blood stasis of Ps applying Liangxuejiedu decoction,Yangxuejiedu decoction and Huoxuejiedu decoction.By this clinical rearch,verify the accurate distiinguision and effective treatment of three famous TCM doctors'experience about Ps. Results1.The analysis of clinical data means. The Ps principally effect the youth from 20 and 40, the male patient is more than the female patient,the ratio is 1.75:1.The patients' nation mainly is Han. Some patients have family history.The disease course is long and easy to fall ill repeatedly.It has obvious cause.The cause mainly is infection.The diagnosement of Western medicine mainly is vulgaris Ps.2.The TCM psoriatic regulation of syndrome distribution and variation.The main psoriatic syndrome differentiations and typings are the syndrome of blood heat,blood dryness and blood stasis. The three syndromes'(blood heat,blood dryness and bloodstasis) distribution and variation are alike in the different sex,age,season and total course(p>0.05). The three syndromes' distribution and variation are obviously different in the different period and present course(p<0.01).The psoriatic progressive period mainly is the syndrome of blood heat,the extinction period mainly is the syndrome of blood dryness,the quiescent period mainly is the sydrome of blood stasis.The short present period is the sign of the syndrome of blood heat and the long present period is the sign of the syndrome of blood dryness and blood stasis.3.The TCM psoriatic regulation of syndrome composement.Most parts of body'ssymptoms'distribution are alike in three main syndromes(blood heat,blood dryness andblood stasis)(p>0.05).But seven symptoms' distribution including throatjipcolour,drinking water,mood,stool,urine and mentrues are obviously different in thethree main syndromes(p<0.01).Four tongue manifestations' distribution including bodyof tongue,crack,trace of tongue and veins under the tongue are alike in three mainsyndromes(p>0.05).Three tongue manifestations' distribution including substance oftongue,fur of tongue and ecchymosis are obviously different in the three mainsyndromes(p<0.01).Four pulse manifestations including slippery pulse,rapidpulse,thready pulse and hesitant pulse are obviously different in the three mainsyndromes(p<0.01).Four pulse manifestations including stringy pulse,deeppulse,normal pulse and other pulses are alike in three main syndromes(p>0.05).Thepatients' lesion information's distribution including new lesions in the past twoweeks,isomorphic reaction and spot bleeding is obviously different in the three mainsyndromes(p<0.01).The lesion shape's distribution including guttata,plaque andrupioides is obviously different in the three main syndromes(p<0.01).The lesioncolour,infiltration and following symptom's distribution are obviously different in thethree main syndromes too(p<0.01).But five items including the whole skininformation,scale coverting erythema,scale information,following symptom and specialbeing involved position are alike in three main syndromes (p>0.05).4.The clinical research.The psoriatic syndrome of blood heat's cure rate,markedly effective rate and total effective rate are respectively 23.3%,56.7% and 80.0% in the treated group.In the control group,the syndrome of blood heat's cure rate,markedlyeffective rate and total effective rate are respectively 16.7%,56.7% and 73.4%.The clinical effect of two groups is alike(p>0.05). The psoriatic syndrome of blood dryness's cure rate,markedly effective rate and total effective rate are respectively 15.0%,50.0% and 65.0% in the treated group.In the control group,the syndrome of blood heat's cure rate, markedly effective rate and total effective rate are respectively 10.0%,40.0% and 50.0%.The clinical effect of two groups is alike(p>0.05). The psoriatic syndrome of blood stasis's cure rate, markedly effective rate and total effective rate are respectively 15.0%,45.0% and 60.0% in the treated group.In the control group,the syndrome of blood heat's cure rate, markedly effective rate and total effective rate are respectively 5.0%,20.0% and 25.0%.The clinical effect of two groups is obviously different(pO.Ol). Conclusionl.The TCM psoriatic regulation of syndrome distribution and variation.In the long process of disease's happening,development and changement,psoriasis is inseparable and stubborn.This disease has recurrent attacks. The three syndromes' distribution and variation are alike in the different sex,age,season and total course(p>0.05). The three syndromes' distribution and variation are obviously different in the different period and present course(p<0.01).The three syndromes exist phase.The beginning of the morbidity is the syndrome of blood heat.Following the extension of the disease progress,the syndrome convert blood dryness and blood stasis.Blood heat is the key point of the pathogenesis's invertion.2.The TCM psoriatic regulation of syndrome composement.The syndrome of blood heat's main clinical manifestations are pharyngitis or pain in throat,red lip,hydrodipsia and cool drink,disturbed and tantrum,constipation,normal urine or yellow and deficient,normal menstrual colour and quantity or crimson and much menstruation,red tongue texture,thin or greasy yellow fur,slippery or rapid pulse,many new lesions in the past two weeks, isomorphic reaction,spot bleeding,erythema being not covered by scale,many guttata lesions,red or crimson lesions,light infiltration of lesionsjight or midrange pruritus,feeling lesions burning. The syndrome of blood dryness's main clinical manifestations are pale red lip, hydrodipsia but not beingdrink,nomal urine and stool, normal menstrual colour and quantity or pale red and little menstruation,pale red tongue,thin and white fur or little fur,thready pulse, few new lesions in the past two weeks,negative isomorphic reaction and spot bleeding, erythema being not covered by scale,many plaque lesions,pale red lesions,light or midrange infiltration,pruritus.The syndrome of blood stasis's main clinical manifestations are dark red lip, hydrodipsia but not being drink,normal mood,nomal urine and stool, normal menstrual colour and quantity or dark red menstruation with coagulated blood,dark red tongue with petechia and ecchymosis,thin and white fur or little fur,thready and hesitant pulse, few new lesions in the past two weeks,negative isomorphic reaction and spot bleeding, erythema being not covered by scale,many plaque and thick shell's lesions,dark red lesions, midrange or serious infiltration,prurita lesions.3.The clinical research.The clinical finding indicates that Liangxuejiedu decoction and Yangxuejiedu decoction can significantly improve the lesion and pruritus of the syndrome of blood heat and blood dryness.The effect of treatment based on syndrome differentiation according to the clinical dortors' experience is alike with the Liangxuejiedu decoction and Yangxuejiedu decoction's effect.Huoxuejiedu decoction and the treatment based on syndrome differentiation according to the clinical dortors' experience altogether can significantly improve the lesion and pruritus of the syndrome of blood stasis.But there is notable difference in the Huoxuejiedu decoction and the treatment based on syndrome differentiation according to the clinical dortors' experience.The clinicians must study and summarize the experience of the famous doctors to reinforce the clinical effece.At the same time,the clinical finding indicates that the effects of Liangxuejiedu decoction , Yangxuejiedu decoction and Huoxuejiedu decoction are satisfacting.The three decoctions have no side effect and are worth to extend in the clinical treatment.lt can provide scientic evidence to the psoriatic treatment through studing the certain treatment mechanism and digging the scientic connotation of the famous dortors' experience.4.The inexorable trend of traditional Chinese medicine's development is the research of the syndrome standardization. The traditional Chinese medicine's treatmentis dominant because of syndrome differentiation-based treatment,abundant methods,actibe medication,good curative effectivejow recurrent rate and long paracmasis.This study's purpose is to approach the TCM regulation of syndrome differentiation including distribution,variation and composement.The study is based on the principles of EBM and the investigation methods of clinical epidemiology.The study expands the thinking way of syndrome research and establishs the foundation of the following large scale syndrome research.At last,the study is benefit to elevate the therapeutic effect and educe the greatest dominance of the traditional Chinese medicine's therapy.
