| Gestational diabetes mellitus(GDM) is a common complication of pregnancy.It affects maternal metabolism that could cause macrosomia or fetal growth restriction,its accurate pathogenesis still not completely clear.But the generally accepted theory is insulin resistance(IR) state exist during pregnancy due to physiological adaptation,and it aggravates when GDM occur.Adiponectin(APN) as a factor of insulin sensitizer was found in recent years that was considered very close with GDM.APN produce a marked effect by conjugating with the APN-receptor(AdipoR) in the target cell membrane.AdipoR play an important role in the procedure of APN-mediated.Placenta as a link connecting mother and child can secrete many kinds of hormones that were significant for their metabolism.Many scholars have found that APN expressed in the placental site,furthermore,the gene was changed in GDM.Whether AdipoR express in the placenta,or what about the expressional type,or how great change of endocrine placenta between GDM patients and normal pregnancy,that has not come to a clear conclusion owing to the less information currently available.Objective:To measure the relative expression level of the placental site APN and AdipoR1/2mRNA in the patients of GDM,and to analyze the relationship in APN and AdipoR1/2mRNA and birth weight and newborn PI.Methods:By the RT-PCR technique,to dectect the expression level of the placental site APN and AdipoR1/2mRNA in 30 normal pregnancy mothers and 26 GDM pregnancy mothers,and to take semiquantitative analysis.Results:(1) APN and AdipoR1/2 mRNA express in the placenta;(2) neonatal birth weight (3673.08±140.16) and neonatal PI(3.20±0.23) in the GDM group were higher than those in the normal control group(3385.83±179.76;3.00±0.25)(P<0.001;P=0.003);(3) the expression level of APN mRNA in GDM group(0.75±0.19) were significantly lower than that in the control group(1.17±0.17),and the expression level of AdipoR1 mRNA in GDM group(2.08±0.67) were no difference than those in the control group(1.98±0.68)(t=0.53,P=0.602),the expression level of AdipoR2 mRNA in GDM group(1.58±0.36) were significantly higher than that in the control group(1.19±0.25)(t=4.76,P<0.001);(4) The expression level of APN mRNA in the GDM group were negatively correlated with birth weight(r=-0.959,P<0.001) and newborn PI(r=-0.477,P=0.014);The expression level of APN mRNA in the control group were negatively correlated with birth weight(r=-0.775,P<0.001) and newborn PI(r=-0.662,P<0.001);The expression level of APN mRNA in the two control groups were negatively correlated with birth weight(r=-0.915,P<0.001) and newborn PI(r=-0.641,P<0.001);The expression level of AdipoR2 mRNA between GDM group and control group were no correlated with birth weight(r=-0.276, r=-0.281,r=0.177;P=0.173,P=0.132,P=0.193),and newborn PI(r=0.143,r=-0.155,r=0.236; P=0.487,P=0.412,P=0.08).Conclusion:(1) There were APN and AdipoR1/2 gene expression in placental tissue;(2) neonatal birth weight and neonatal PI in the GDM group were higher than those in the normal control group(P<0.05);(3) the expression level of APN mRNA in GDM group were significantly lower than that in the control group,and the expression level ofAdipoR1 mRNA in GDM group were no difference than those in the control group,the expression level of AdipoR2 mRNA in GDM group were significantly higher than that in the control group;(4) The expression level of APN mRNA between GDM group and control group were negatively correlated with birth weight and newborn PI,The expression level of AdipoR2 mRNA between GDM group and control group were no correlated with birth weight and newborn PI. |
