A City Different Levels Of Hospital Imaging And Testing Equipment In The Status Quo Analysis And Configuration Research

BACKGROUD:With the rapid development of science and health technologies,new health equipments emerge.WHO held a consultation meeting on the creation of list of essential health technologies to help the member states to strengthen the plan and use of health technologies in 2007.Health equipments play a great part in diagnosis and therapeutics of disease in our country.The uneven and unreasonable distribution of health equipments could not fit for the ideal health service.It benefits us to analysis the current situation of health technologies from different levels of health care facilities in aspects of understanding the current situation and problems of health technologies allocation.The suggestions will be made to enhance the allocation and management of health technologies.OBJECTIVES:The study aims to describe and analyze the current situation of some health technologies from different levels of health care facilities in a city,and to make essential health equipment lists for different departments.Last but not least,a proposed model of determinants is established.What's more,the research group provides some suggestions for the government to formulate the relevant policy in order to strengthen management,allocation and use of health technologies.METHODS:The researchers collected the information of 10 sampling hospitals through the typical survey.The structured questionnaire was designed on basis of WHO Global Action Guideline combined with current situation.It includes information of hospitals, some health technology allocation,human resource and equipment maintenance.It was filled by the head of related department.In the process of focus group discussion,medical experts and health managers discussed health technologies allocation of 10 hospitals and expressed their thoughts about the list for different levels of health care facilities.The research group tried to formulate essential health equipments list for some departments.In order to find determinants of equipment allocation for X-ray machine and blood cell analyzer,in-depth interviews were carried out.Quantitative data was input into and analyzed by EXCEL 2003.Focus group discussions were recorded by tape and collected by shorthand.The result was classified and analyzed according to topics.The result of in-depth interview was given value,calculated the means and standard deviations for the determinant ranking. RESULTS:1.The majority of image equipments in sampling hospitals were in good operation.Those were mainly maintained by suppliers and themselves as supplementary.The differences exist in maintenance and preventive maintenance between different hospitals.2.For previous education,most medical personnels in imaging departments in tertiary hospitals are bachelor degree holders.Medical personnels in secondary hospitals and primary hospitals mainly have junior college degrees.Some personals have never received medical imaging education.The majority of medical personnels received on-job training and post-job training,and also passed the qualification examination.3.The top three clinic laboratory equipments in all levels of hospitals are blood cell analyzer,clinical chemistry analyzer and urine analyzer.The differences exist in status of operation,maintenance and preventive maintenance between different hospitals.4.For previous education,most medical personnels in clinic laboratory departments in tertiary hospitals are bachelor degree holders.Medical personnels in secondary hospitals and primary hospitals mainly have junior college degrees.The majority of medical personnels received on-job training and post-job training.5.In sampling hospitals,equipments in imaging departments meet the needs in all level of hospitals.Essential imaging equipment list for economical developed area was made through focus group discussion.For determinants of X-ray machine,image clarity, image amount per day and number of patients play a greater role.6.In sampling hospitals,equipments in clinic laboratory departments meet the needs in all level of hospitals.Essential clinic laboratory equipment list for economical developed area was made through focus group discussion.For determinants of blood cell analyzer,precision,test speed and creditability of the abnormal morphologic warning messages play a greater role.7.In the proposed determinant model,physicians' demand,patients' needs and hospitals are the predisposing factors.Suppliers,equipment features and purchasing power are the enabling factors.Government regulation is the reinforcing factor. SUGGESTIONS:The following are the suggestions proposed according to the research.1.To formulate the basic standard of health equipment allocation and management based on the current situation.Adhering to the principle of "scientific rational comprehensive consideration" while formulating the standard.To dynamically manage and regularly revise the standard.2.To strengthen policy guidance.To promote professional personnel introduction,training and management.3.To reinforce the ability of equipment maintenance.To pay attention to preventive maintenance.And to cultivate maintenance labors.Last but not least,to adopt diverse cooperative maintenance models.
