| Objective:To evaluate the therapeutic effects and complications of threedim-ensional conformal radiotherapy (3DCRT) for non-smell cell lung cancer (NSCLC) Methods:107 patients who were treated either by radiotherapy alone (26patients) or radiotherapy plus chemotherapy (81 patients) from Jan.2005 to Feb.2008 were entered into the database for analysis. Patological types:squamous cancer (56), adenocarsinoma (43), squamoadenocarcinoma (2) and other type (6). stages:11 had I, 10 had II,56 hadâ…¢and 30 had IVstage diseas. This patient cohort consisted of 75men and 32 women. The median age was 62year. radiotherapy was delivered at 2Gy fraction, 5 fractions per week. The median total dose 60Gy. Reasalt:With a median follow-up time of 27months, the response rate after 3DCRT was 57.1%, with complete remission 11. 0%(10/91) and partial remission 46.2%(42/91). The 1-,3,-5-year overal survivals were 67%,31%,22%, The median surviva time was20.2months, By logrank test, clinical stage, KPS perfomance, tumor volume, consolidation chemotherapy, radiation dose and response to treatment showed statistically dramatic impact on overall survival. in Cox multivariable regression, The independent adverse prognostic factors univariate analysis and multivariate analysis was clinical stage, treatment type, response to treatment. Grade 2 acute radiation pneumonitis was observed in 3 patients and grade 3 in 2 patients. Late lung injury developed in 1 patient with grade 2,1 patient with grade 3, Acute radiation esophagitis was observed in 49 patients with grade 1. above Grade 2 developed in 9 patients. Acute grade 1+2 hematologic toxicity developed in 32 patients. above Grade 2 developed in 5 patients. Conclusions 3DCRT was feasible in the treatment of NSCLC with good immediate tumor response and acceptable normal tissue complication. The total dose may potentially be increased. |
