Survey Of Prevalence And Behavioral Risk Factors For AIDS/STD Among Female Sex Workers In Tianjin

Objectives:To understand the present situation of AIDS and STD infection among female sex workers in our city, analyze the relationship between sexually transmitted infection status of AIDS in this population and their behavioral risk factors, and to provide scientific foundation for relevant departments establishing comprehensive AIDS/STD prevention and control strategy for female sex work, we specifically carry out this study.Methods:By making cross-sectional study, using non-probability sampling method (convenience sampling) determining the research field, all adopting face to face interview survey method, under the condition of obtaining the informed consent, conduct on-site interviews using given survey questionnaire for the group. Specific research studies include:1. By using self-drafted general survey questionnaire, making research on the group's main socio-demographic characteristics and behavioral characteristics, sexual behavior, and other related high risk behaviors of HIV/STDs infection, related knowledge on HIV/STDs and behavioral characteristics of seeking for medical attention, etc.2. Detect the infection conditions of HIV, sexually transmitted infections (including HPV, Chlamydia trachomatis, Neisseria gonorrhea, syphilis) for female sex workers. Based on obtaining female sex workers' HIV/STDs infection rate, analyze and discuss the related risk factors in influencing female sex workers' HIV/STDs infections.3. Use SPSS12.0 statistics software for statistics analysis on the research data. By adopting the non-conditional single factor Logistic regression in cross-sectional survey, analyze related influence factors in female sex workers' HIV/STDs infections. In the selection of the variables, the standards for variables entry and exclusion models are set to be p <0.05 and p> 0.10 respectively. Parameters'OR value in the calculation model and 95% CI (Confidence interval, CI). Models are tested by goodness of fit test.Results: 1. The average age among female sex workers are 27.017.76 years old, and the median age is 24.00 years old. The people from 20-29years old accounts for 68.8%; the Han nationality accounts for 94.4%; education level is low, and junior middle school education level accounts for 54.9%; single status accounts for 65.6%; non registered permanent residents account for 97.7%, mostly from northeastern provinces which occupies 44.65%; female sex workers concentrated in middle-level entertainment places account for 63.3%,28.8% from low-level places, only 7.9% from high-end places. Sex workers in our city are highly mobile population,60.0% staying in less than half a year will flow to other provinces, or move from other provinces to Tianjin.2. The female sex workers survey were not detected infected with HIV-positive. Other sexually transmitted infections were:27.0% for HPV infection,10.2% for syphilis infection,7.4% for Chlamydia trachomatis infection,4.7% for Neisseria gonorrhea infection,0.9% for HCV infection rate,43.7% for STDs (except HIV, any one of the sexually transmitted infections is STD infection)3. Among female sex worker group, they largely have wrong perceptions on AIDS/STD prevention and control knowledge. Among the eight questions on the related AIDS prevention and control knowledge, answering 7 or above questions correctly are viewed as awareness. The general awareness rate on the knowledge of AIDS prevention and control among female sex workers is 58.6%; for four questions on STD prevention and control, only 28.8% gave the all correct answers. In the AIDS prevention and control section, for the question that if mosquito bites spread AIDS, the correct answer rate is the lowest which is only 61.4%; In the STD prevention and control section, for the question that if vaginal douching or taking antibiotics can prevent STDs, there was wrong perception, the correct answer rates were 41.9% and 45.1%.4. In the survey, the rate of using condoms by female sex workers in the most recent commercial sex behavior was 81.9%. For the commercial sex behaviors in the recent past one month,42.3% of female sex workers answered that they used condoms every time. The consistent usage rate was less than half.57.2% of the group answered that they used condoms sometimes, and there was one female sex worker answering that she never used condom. Therefore, the condom usage rate is relatively low. For all the female sex workers surveyed, none admitted that they used drugs. For the FSW group surveyed, in the past 12 months, the maximum number of commercial sex trade was four for one night; the minimum number was 1-2.5. Among female sex workers, the intervention coverage rate is still very low, the frequency and depth of promotion and intervention is not enough. Female sex workers surveyed this time all heard about AIDS, television was the recourse for most of them knowing about related information on AIDS, which was about 98.1%; 56.3% of the group obtained AIDS-related knowledge from AIDS prevention and control promotion materials; 4.7% from newspapers and magazines; Another 1.9% from friends. In the group surveyed,54.9% received condom promotion and distribution/intervention services of AIDS counseling and testing,3.3% also received peer education; 17.2% received HIV counseling and testing in the past one year, and 16.7% knew the testing results.Conclusion:The infection rate of STDs among female sex workers is high; the group has low awareness of AIDS/STDs prevention and control knowledge, the channel promoting AIDS prevention and control knowledge is limited; condom usage rate is still relatively low, the frequency of the intervention measures and coverage is not sufficient. Take positive and effective measures to reduce the variety of risk behaviors among female sex workers population, controlling the epidemic spread of HIV/STDs in this group is important to reduce the infection rate of HIV/STDs infection in the population in China, which is an important part of protecting human health.
