| Objective:To compare the clinical effect and evaluate the percutaneous access of upper urinary calculi with minimally invasive percutaneous nephrolithotomy under the guidance of B-ultrasound or the X-ray.Methods:60 cases of renal calculi underwent minimally invasive percutaneous nephrolithotomy under the guidance by B-ultrasound or the X-ray.30 cases(18 males,12 femals)were guided by the X-ray and the others(19 males,11 femals)by B-ultrasound. The mean puncture time, mean bleeding volume, hospital stay, rate of stone residual and complications were compared.Results:The B-ultrasound group:The mean puncture time was 8.5±5.2minutes (5.0-12.1min).The rate of operative complication was 6.67%(2/30).The rate of residual stone was 10%(3/30).The mean hospital stay was 5.8±2.0d(4-10d).The mean volume of bleeding was 45±10.2ml(30-120ml)during operation. The X-ray group:The mean puncture time was 13.2±4.4min(7.1-19.2min).The rate of operative complication was 10.00%(3/30).The rate of residual stone was 13.33%(4/30).The mean hospital stay was 6.2±2.5d(5-15d).The mean volume of bleeding was 50±12.0ml(40-100ml)during operation.Conclusion:PCNL under the guidance of B-type ultrasound and X-ray is safe and effective procedure of calculus in upper urinary.The operative time of X-ray group was more than the B-ultrasound group.The different way can be used for patients and operators in accordance with the different situation. |
