Objective:According to the evaluation of the clinical effect and safety of integrated Chinese and Western medicine on hypertensive patients in the elderly, to clarify the definite effect and safety of integrated Chinese and Western medicine. On that basis, to research the antihypertensive effect and the improvement of life quality of JiangYa granule combined western medicine on hypertensives in the elderly.Methods:1.Computer retrieval of the Pubmed, Cochrane library, CNKI,VIP, and Wangfang databases, searching for the literature of randomized controlled trials on the treatment of integrated Chinese and Western medicine on hypertensives in the elderly. We used the standards of methods evaluation recommended by the Cochrane Handbook 5.0.2 to strictly evaluating quality of the literature, and the results of the trials were given meta-analysis by the Revman5.0.2 software.2. Using the method of computer randomized and positive medication controlled, 130 cases of hypertensive which met the inclusion criteria were randomly assigned to 2 groups,65 cases in the control group, given Nifediping sustained release tablets and Enalapril maleate tablets, and 65 cases were assigned to the treatment group, which given Jiang Ya granules on the basis of the treatment given in the control group. The total treatment period was 6 months, and to observe the antihypertensive effect, improvement of life quality as well as the medication safety of both groups.Results:1.After the systemic analysis, we found that the total relative anti-hypertensive effect in the integrated group varied as the different selection of antihypertensive drugs in the Western medicine group, but no worse than the Western medicine group as a whole. At the aspects of improvement of life quality and clinical symPtoms, the integrated treatment played an important role, obviously better than the Western medicine group, especially on the relief of the symptoms of dizziness, headache and palpitation, hence greatly improved the patients' quality of life. However, on the aspect of method quality of the literature, strictly designed, large samPles and double- blinded randomized controlled trails were needed to provide high quality evidence, so as to guarantee the real and objective of the research results, and make the evidence more persuasive.2. Combined with western medicine, JiangYa granule could significant decrease the Patients'systolic blood pressure on the clinic blood pressure, better than the control group(P<0.05); As to the comparison of the 24 hours ambulatory blood pressure, there were significant difference before and after treatment in both groups (P<0.05),and both had reached the normal level of blood pressure, but there was no statistical difference in the comparison between the two groups (P>0.05);the treatment group was better than the control group on the decrease of night systolic blood pressure load, which had statistical significance(P<0.05); and the total antihypertensive effective rate were both above 98%. With regards to the syndrome scores of traditional Chinese medicine, the treatment group were significantly better than the control group not only on the scores after the treatment but on the score differences between before and after treatment, and had significant statistical difference(P<0.01); Except for the sexual disorders and anger factors, the rest 9 factors of Du Shi hypertensive life quality questionnaire in the treatment group were all lower than the control groupafter treatment (P<0.05); on the improvement of symptoms in Symptom Checklist 90, compared to that before treatment, factor scores in the treatment group were all lower (P<0.05);while in the control group, factors of depression, terror, stubborn, psychosis and others had no significant difference compared to those before treatment (P>0.05); Compared to the control group, all factors in the treatment group were better except for the factors of terror and stubborn. As to the drug safety, the incidence of adverse reaction in the treatment group was lower than that in the control group, and there were no abnormal changes in blood routine, urine routine, liver and kidney function, as well as electrocardiogram in both groups before and after treatment.Conclusion:The antihypertension of the integrated Chinese and Western medicine treatment was effective and safe, confirming the evaluation of effect of integrated Chinese and Western medicine on hypertensives in the elderly, and could greatly improve life quality of the patients, worthy of clinical popularization and application.
