Clinical Research On Therapy Of Tonifying Kidney, Clearing Heat And Removing Stasis In Treating Type 2 Diabetic Nephropathy

Purpose:To discuss the understanding of traditional Chinese medieine about type 2 diabetic nephropathy, using the method of tonifying kidney,clearing heat and removing stasis.To observe the therapeutic efficacy of this method in treating the syndrome stagnated heat of diabetic nephropathy.Methods:A randomized, controlled trial was conducted 50 patients with type 2 diabetic nephropathy were randomly classified into two groups.On the basic of normal treatment and western medieine, the patients of treated group (n=25) were treated by chinese medicine while the patients of controlled group (n=25) were not.Three months was a course of treatment.The observed indexes included both curative effect indexes,such as clinic symptoms,FBG,PBG,HbA1c,bloodpressure,NAG,24hUTP,SCr,BUN.bloodfat,haemotheology and CRP.The complete data carries on statistics analysis by the SPSS17.0 statistics software.Results:Prior treatment,the gender,age,course of disease of the two groups patients who were confirmed and incorporated had no differeces.①The total effective rate of treated roup patients was 88.00%;while the controlled group was 64.00%.The treated group had an advantage than the controlled group (P<0.05).②The blood glueose,the blood pressure,HbAlc of two groups lowered (P<0.01).And the treated group had an advantage than the controlled group (P<0.05).③The NAG of treated group had an obviously reduce(P<0.01), which had an advantage than the controlled group(P<0.05).④The 24h UTP of treated group had an obviously reduce(P<0.01), which had an advantage than the controlled group(P<0.05).⑤After treatment,SCr,BUNdecreased significsntly,which improued compared with pre-treatment(P<0.01).⑥Compared with pre-treatment, in the treated group Patients' TC,TG,LDL-C had a lowering and HDL-C had marked inerease (P<0.01),showing that the combination of western medicine and Chinese medicine has the function of adjusting fat and it does much better than using western medicine effected in TC,TG,LDL-C and HDL-C.⑦After treatment, the whole blood viscosity (highly cuts,middlely cuts,lowly cuts5/s),the blood plasma viscosity,RBC aggregation index and hematocrit of the treated group droped.The dropping of whole blood viscosity(highly cuts,middlely cuts) between treated group and controlled group had a difference (P<0.05), the blood plasma viscosity had a rernarkabledifference(P<0.01),but the drop level of whole blood viscosity(lowly cuts5/s), RBC aggregation index and hematocrit between the two groups had no obvious difference.⑧The CRP of treated group had an obviously reduce, which had an advantage than the controlled group(P<0.05).⑨Safety indicators:blood,urine,stool with fecal occult blood test ECG,the liver and kidney function tests were within the normal range.Conelusion:Through the preliminary clinical study, Chinese medicine of method in tonifying kidney,clearing heat and removing stasis has sure curative effected in treating diabetic nephropathy.It is worth studying deeply and developing further.
