Objective:To explore the application of intra-aortic balloon pump(IABP) in the patients who were low cardiac output syndrome and severe pulmonary hypertension after cardiac valve replacement.Methods:A retrospective analysis from October 2009 to March 2011 22 patients with severe valvular disease of the clinical data. Application of IABP in patients after valve surgery in 10 patients (groupâ… ),the control group (Groupâ…¡) IABP was not applied for the same period in patients after severe valve disease in.12 cases. Observation of hemodynamics, cardiac function, heart rate, the early survival rates were compared.Results:IABP assist time was 28-372h, in the application of IABP patients, the radial artery pressure in the early decline, increased diastolic pressure, mean arterial pressure in the application after it is improved significantly reduced peripheral resistance, pulmonary arterial pressure decreased. Changes in arrhythmia after the application of IABP frequency or ventricular tachycardia PVCs into sporadic PVCs, ST segment elevation or lower in the 1-2 hours after the return to normal. Selected cases,8 males and 14 females, aged 41 to 75 years. Anesthesia cardiopulmonary bypass were valve replacement, mitral valve replacement in 8 cases in which,7 cases of aortic valve replacement, mitral and aortic valve replacement in 7 cases,29 were implanted valve, mechanical valve 22 Pieces (imported machinery flap 18, made mechanical valve 4), biological valve 7, transit time of 45-307 min, blocked 29-168min, length of stay of 12 to 67 days. Group I patients died 0 cases,4 died during the postoperative hospital stay, early mortality 40%. Group II patients died 0 cases,10 died during the postoperative hospital stay, early mortality 83%.Conclusion:choose the appropriate timing of surgery, the full preoperative preparation, improve heart function, intraoperative exercise caution, good myocardial protection, postoperative intensive care, timely application of IABP and other assistive devices in patients with severe valvular line the survival rate after valve replacement Improve the apparent results.
