ObjectiveTo observe the process of fracture healing in diabetic rats and the variation after GLP-1 intervation treatment. Then to investigate the possible factors related to frature healing in diabetic rats and treatment of GLP-1.MethodsTwo hundred and ten male Wister rats were randomly divided into three groups.Group A was Diabetes GLP-1 treatment group,Group B was non-treatment Diabetes group,Group C was control group. In group A and B , alloxan was injected into their abdominal cavities to make dibete mellitus model,in group C equivalent sterilized saline was injected. Fracture model was made in tibial and then reducted and internal fixed. At 0,48 h,3d,1 and 2,4,6,8 weeks after operation blood of glucose was measured. The X-rays of tibial taken in the three groups in 1th,2th,4th,6th,8th week after the surgery to evaluation the fractue healing. After 1,2,3,4,6,8 weeks operation the serum IGF-1 was examined by ELISA assay.1,2,4,6,8 weeks after fracture, 5 rats were sacrificed in each group to observed the growth of callus, the tissue of fracture region was taken for HE staining. At 4th, 6th and 8th week after operation biomechanical test was done. All the data was processed by the SPSS11.5 statitical package.ResultsAfter alloxan injection rat's blood glucose were significantly increase in Group A and B. Blood glucose in group B all exceeded 16.7mmol/l .There was no significant in the blood glucose level between Group A and Group B after GLP-1 intervation. Antomic observation showed that every time point group B callus growth was slower, there was no significant difference betweer group A and group C.There were more periosteal reaction and rapid bone reconstruction in group A and group C than in group B .The diabetic callus mechanical strength was significantly lower than that in control group and GLP-1 therapy group. HE staining suggested the chondrocyte maturation hysteresis and hypertrophy in diabetic rats.Serum IGF-1 level were significantly decreased and the peak was delayed for one week in dabetic rats.ConclusionThe diabete mellitus severely affected the rat fracture healing , and the delayed and non-union occurace was significantly increased. The level of expression of IGF-1 in serum was decreased and peak was delayed in diabetic rats and the GLP-1 therapy could improve their expression significantly.
