ObjectiveThe objective of this project is to investigate the incidence and risk factors of high birth weight in Changzhou city of Jiangsu province.And the result is to provide a scientific basis for reducing the incidence of high birth weight and improving infant health and finally the health level of general public.MethodsWe used a Cross-sectional study to analyze sample of 6,251 subjects randomly seclected from cities, towns and villages in Changzhou. 6,251 babies who were born during Jan 1st, 2006 to Dec 31th, 2008 and their parents were enrolled in our survey. The contents of investigation were including: general condition of infants (birth place, age, gender, nation, child-order, and feeding-history) and their parents(name, age, occupation, home-revenue et al), health condition in pregnancy of mother(abortion and artificial labor of mother). Univariate and multivariate logistic regression were used to evaluate odds ratios (ORs) and 95% Confidence Interval (α=0.05).ResultsAmong 6,251 samples, 26 were deleted because of incomplete information.Valid sample size is 6,225. The incidence of low birth weight was 2.36% (147/6225 persons). The incidence of high birth weight was 11.60%(722/6225 persons). Rate of normal birth weight was 86.04% (5356/6225 persons). Birth place, gender and mothers'childbearing ages between the group of high birth weight and the group of normal birth weight were significant difference(P<0.016-0.001). There were no significant difference in year, mothers'education level, parity and times of prenatal check-ups between the two groups(P=0.234~0.961).Infant and child mortality in high birth weight group was 0.88%, which was higher than normal group(0.28%)(P=0.016). Compared to normal birth weight group, the risk of infant and child mortality in high birth weight group was higher (OR:3.033, 95%CI:1.173-7.843).Univariate logistic regression analysis showed that boy, town of birthplace, post-mature infant and mother's childbearing age of and over 35 were statistically significant, and the ORs (95%CI) were 1.795(1.524-2.114), 1.292(1.080~1.541), 1.737(1.125~2.681), 1.382(1.114~1.715) respectively.In order to control confounding factors, adjusted sex, region, mothers'educational level and age, multivariate logistic regression analysis showed boy, town of birthplace,post-mature infant and mother's childbearing age of and over 29 were related to high birth weight ,and ORs were 1.889(1.564~2.282), 1.469(1.146~1.883), 1.355(1.026~1.790), 1.569(1.193~2.063) respectively. The result suggested the 4 factors were independent risk factors for incidence of high birth weight in Changzhou City.Analysis of risk factor's gather were execute, the factors included gender, town of birthplace, post-mature infant and mother's childbearing age. Multivariate Logistic regression analysis showed the risk of high birth weight increased accompanied gather of numbers of risk factors, and there was obvious dose-response relationship (trend test, p<0.001).Conclusion1. The incidence of high birth weight in Changzhou was 11.6%, which was higher than China (6.3%).2. Infant and child mortality in high birth weight group was higher than normal group.(P<0.001), the OR (95%CI) was 3.033(1.173~7.843). High birth weight was one of important risk factors that cause infant and child mortality in Changzhou.3. Single-factor and multi-factor logistic regression showed that the important risk factors were boy, town of birthplace, post-mature infant and mother's childbearing age of and over 29.4. The risk of high birth weight increased accompanied gather of numbers of risk factors in Chouzhou.
