Objective To explore the changes and clinical significance of serum high-mobility group box 1 protein (HMGB1) and hypersensitive C-reactive protein (hs-CRP) levels in patients with coronary heart disease (CHD), to reveal the relationship between inflammatory and CHD, and in order to provide its possible new evidence of prevention, diagnosis, risk stratification, treatment and prognosis prediction.Methods The serum HMGB1 and hs-CRP were determined in 20 acute myocardial infarction (AMI),20 unstable angina pectoris (UAP),20 stable angina pectoris (SAP) by enzyme-linked immunosorbent assay (ELISA) and scatter velocity turbidimetry respectively, and compared with 20 healthy subjects.Results The serum HMGB1 and hs-CRP were significantly higher in CHD patients than that of the control group (t'=8.02,8.43, P<0.01). The concentrations of serum HMGB1 and hs-CRP in AMI,UAP and SAP were significantly different。The serum HMGB1 and hs-CRP in patients with AMI were obviously higher than that of UAP, SAP and the control group (F=31.48,24.60,q=4.32~12.84, P<0.01); and that in patients with UAP were much higher than that of patients with the SAP and the controls (q= 3.98~8.51,P<0.01); and there were statistical differences between SAP and the controls (q=3.63,2.90, P<0.05). There was a positive correlation between the serum HMGB1 and hs-CRP (r=0.287, P<0.05). The areas under the ROC curve (AUCROC) were 0.98 and 0.97 in AMI,0.95 and 0.94 in UAP as well, with higher sensitivity and specificity. The sensitivity and specificity of HMGB1 and hs-CRP were 0.90,0.95 and 0.85,0.95 in patients with AMI. The sensitivity and specificity of HMGB1 and hs-CRP were 0.95,0.85 and 0.90,0.90 in patients with UAP.Conclusion The changes of HMGB1 and hs-CRP levels have a great value and higher accuracy on the diagnosis of AMI and UAP, which might be used as one of the indexes to evaluate the degree of severity of CHD.
