| Correlation Study between the Changes of the Liver Hemodynamics and the Gallbladder Wall tested by Color Doppler Ultrasound and theLiver FunctionObjective:To observe the hemodynamic changes of doppler waveform in portal vein, hepatic vein and artery for the patients with disseminated liver disease, the change of the gall-bladder wall and study the relationship between these changes and liver function.Methods:We collected 32 healthy people,92 cases with fatty liver,30 cases with chronic hepatitis and 28 case with liver cirrhosis. The internal diameter and the rate of pulse of portal vein (PR=minimum of flow rate/maximum of flow rate), the doppler waveform of the right and the middle hepatic vein, the RI (resistance index) of hepatic artery, the wall thickness and the smoothness of the gallbladder were observed by Toshiba aplio770 and Philips HD11 color Doppler ultrasonoscope (probe frequency2-4MHz).The participants are kept fasting for at least 8 hours before the examination.Results:1:Fatty liver group:There was no significant statistic difference between mild, moderate and the control group, but there was significant statistic difference in the hemodynamics changes between severe and other groups.2:Chronic hepatitis group:There were significant differences in the hepatic vein waveform, the diameter and the rate of pulse of portal vein, the RI of hepatic artery and the change of the gallbladder wall in the patients with liver cirrhosis compared with control group.3:Liver cirrhosis group:The hepatic vein waveform, the diameter and the rate of pulse of portal vein, the RI of hepatic artery and the change of the gallbladder wall in the patients with liver cirrhosis has significant differences compared with control group.Conclusion:Ultrasound helps the doctors to detect the variation of portal vein, hepatic artery, hepatic vein as well as the change of the gallbladder wall of the patients with chronic liver disease by observing the hemodynamics of the liver and the gallbladder wall. There was significant difference in the hemodynamics between severe and mild, moderate, the control group. In the group of chronic liver disease, there were significant differences in the hepatic vein waveform, the diameter and the rate of pulse of portal vein, the RI of hepatic artery and the change of the gallbladder wall compared with control group. Therefore it can be used as one of the reference index for early clinical diagnosis and treatment, and it is valuable in evaluating the severity of disseminated liver disease. |
