Objective: With the aging of society growing and living standardsimproving, cardiovascular disease morbidity grows, which seriously poses athreat to human health. In the field of cardiovascular disease, coronaryatherosclerotic heart disease (CHD) has a very large proportion. Unstableangina is a medical emergency. Positive anti-ischemic treatment can improveclinical ischemic symptoms, prevent myocardial infarction and improvepatients' prognosis. A growing number of studies have found that goodtherapeutic effects of Rho kinase inhibitors in cardiovascular disease, butresearches on unstable angina pectoris is still in scarcity. This study is toevaluate the clinical effect and safety of Rho kinase inhibitor-Fasudilhydrochloride, in patients with unstable angina.Methods: In this experiment we chosen90cases with unstable anginaadmitted in our hospital between January2011and December2011.90cases were randomly divided into two groups respectively. The treatmentgroup had45cases (30males and15females, aged40to84yearsold, average59.0years old), while the control group had45cases(27males and18females, aged38to85years old, average61.9years old). Bothgroups were given conventional treatment with nitrates, beta-blockers, aspirin,clopidogrel, low molecular weight heparin, angiotensin-converting enzymeinhibitors/angiotensin II receptor blockers, statins,trimetazidine andother treatments. The treatment of sublingual nitroglycerin were given whenanginal attacks. The treatment group was administrated with fasudilhydrochloric injection (30mg in5%glucose injection or saline100mL)intravenously, twice daily. Both groups were treated for14days. To record thetimes and duration of angina attack before and after the treatment. 12-lead resting ECG was measured and compared with ST-T changes. Torecord ECG whenever angina attacks. Blood pressure measurement wascarried out before and after treatment. Drug adverse reactions were recordedin the treatment duration; Blood routine, urine routine, stool routine, liverfunction and renal function were measured before and after the treatment.Alldata is processed and statistically analysed in SPSS13.0software. P <0.05signifies statistical significance.Results:1Compared with pre-treatment:The frequency and duration of angina attack have no statisticallydifferences between the treatment group and control group (p>0.05).2Compared pre-treatment with post-treatment:The frequency and duration of angina attack decreased after treatment inthe two groups (P <0.05).3Compared with post-treatment:The two groups have good therapeutic effect on angina symptoms andECG. The treatment group administrated with Rho kinase inhibitor(fasudil)hasa better therapeutic effect than the control group (p <0.05).4Compared of the treatment group and the control groupThere is no significant difference in blood pressure between the twogroups before and after treatment(p>0.05). But the blood pressure droppedafter treatment in both groups (p<0.05).5During the period of the treatment,16cases with angina repeatedattacks underwent percutaneous coronary intervention (PCI) treatment in thecontrol group, and14cases in the treatment group.6Adverse reactions:In the treatment group,1case with hypertension administrated fasudilhydrochloride appeared dizziness and low blood pressure symptoms. Thesystolic blood pressure measured was110mmHg. Adjusting antihypertensivetreatment, dizziness was improved and fasudil hydrochloride applicationcontinued; The treatment group had3cases stopping the administration of β-blockers because of heart rate <50beats/min, while the control group had4cases; Each group had3cases stopping the administration of nitratesbecause of insupportable headache.Conclusion: Rho kinase inhibitors combined conventional anti-ischemiadrugs (nitrates, β blockers, et al.) can significantly reduce the frequency andduration of angina attack in patients with unstable angina pectoris.Rho kinaseinhibitors-fasudil hydrochloride, has no significant effect on blood pressure inpatients with unstable angina pectoris, and it is a safe and effective drug forthe treatment of unstable angina pectoris. Rho kinase inhibitors-fasudilhydrochloride is a safe and effective drug for the treatment of unstable anginapectoris,meanwhile it has no significant effect on blood pressure in thesepatients.
