Comparison Of Effect For Calcaneal Fractures By Using The Methods Of PPFK And ORIF

Objective:using percutaneous poking fixation with Kirschner wire (PPFK) and open reduction Internal fixation with plate(ORIF) two surgical methods to treat intra-articular calcaneal comminuted fracture.The study aims to compare calcaneal fracture incidence of complications, calcaneus corrective effect of each parameter, the time discharged from hospital, the recovery of ankle joint activity and functional score.It would provide more scientific programs for clinicians and maximize the quality of life of patients.Materials:and research methods:40cases from (January2010to January2012) the Second Hospital of Shanxi Medical Bone Disease Branch minimally invasive group, were treated (the sanders Ⅱ type) divided into two groups, respectively, PPFK20cases and ORIF20cases.An average age of was about36years old in the two groups, had excluded people with severe cardiovascular disease, diabetes and other cases. Preoperative patients with limb elevation brake,do the calcaneus lateral and axial ray examination,CT scan and measure the calcaneal width, Bohler angle, Gissance, angle and other parameters.After surgery calcaneal parameters of the correction, MFS scoring system function score were compared in the two groups, and then the relevant statistical tests and analysis,P<0.05was considered statistically significant.Results:1There was no significant difference about height, width, the the Bohler corner. Gissane angle before treatment(P>0.05).2By PPFK and ORIF therapy,Bohlers and Gisanse angle significantly restore normal range.There was statistically significant between before and after treatment respectively.(P<0.05).3There was no statistically significant between one day and1year postoperatively in both groups (P>0.05).4Hospital stay percutaneous was6.26±3.13days in the PPFK group,and was19.75±11.27days in the ORIF group, there was statistically significant (P<0.05).5In the PPFK group,35.0%excellent,50.0%good,10.0%acceptable,5%poor, Maryland score82.5±21.3points;In the ORIF group,40.0%excellent,45.0%good,10.0%acceptable,5%poor,Maryland score80.9±30.1.There was no significant different about efficacy assessment and Maryland score in the two groups (P>0.05). 6In the PPFK group, abnormal was5.0%, traumatic arthritis was5.0%, pain was5.0%, complications of a total of15.0%; In the ORIF group,abnormal was5.0%, the traumatic arthritis was5.0%and pain was5.0%, fat bone tendon inflammation was5.0%, complications, a total of20.0%, there was not statistically significant (P>0.05).Conclusion:1two surgical plan can corrected the calcaneus of each parameter to almost normal.2In the PPFK group,patients discharged relatively earlier, to resume their daily work, daily activities,and functional scores was higher,but there was not statistically significant.3In the PPFK group,the incidence of wound complications was relatively low, but there was no significant difference between. PPFK and ORIF group.4The PPFK method was more scientific to treat calcaneal fractures,be worthwhile to promote clinical practice.
