BackgroundRespiratory failure of chronic obstructive pulmonary disease is a serious hazard to human health, respiratory disease is the leading cause of death worldwide, ranking of various diseases forefront of the average cost of hospitalization. COPD patients with respiratory failure, morbidity and medical costs increase, a heavy burden to families and society to improve the prevention and treatment of COPD patients with respiratory failure, control of their health care costs has become an important subject in the respiratory field and the field of health management.The clinical path is a standardized method of diagnosis and treatment to shorten the average length of stay, reasonable pay for medical expenses is characterized by reasonable arrangements hospitalization time and cost of disease to design the best treatment and care programs, according to the disease. It has been proved that clinical pathway not only can effectively reduce the length of stay and hospital costs, but also can significantly improve the quality of medical services by the national medical attention, since the20th century, a new medical model.This research through a historical control study to prove that the implementation of the COPD patients with respiratory failure in Integrative Medicine clinical pathways, the possibility of shortening the total and ICU (Intensive Care Unit) length of stay, reduce patients’average hospital costs, improve patient7days survival rates, improved patient outcomes, to make an objective assessment of the efficacy of clinical pathway to provide an objective basis for the promotion of Integrative Medicineclinic clinical Pathway in COPD patients with respiratory failure.Objectives To evaluation of "Respiratory Failure of Chronic Obstructive Pulmonary Disease" clinical pathways of integrated traditional chinese and western medicine.MethodsThrough the historical control study approach, the study select the pa tients whom the first diagnosed as "chronic obstructive pulmonary disease" and "respiratory failure" be in hospitalized in ICU of Guangdong Provincial Hospital of TCM from January2008to December2009as non-clinical pathway group, and the patients whom the first diagnosed as "chronic obstructive pulmonary disease" and "respiratory failure" be in hospitalized in ICU of Guangdong Provincial Hospital of TCM from January2010to Octorber2011as clinical pathway group, compared with two groups of hospitalized patients with major relevant circumstances. The main evaluation indicators:such as length of stay, hospital costs, seven days survival rate, and so on.ResultsThe study collects180cases in line with the inclusion criteria, including The clinical pathway group of90cases, and the non-clinical group of90cases. The total length of stay from19.31±6.830days down to16.46±7.358days (P <0.01), the ICU length of stay from8.73±3.075days down to7.11±4.803days (P<0.01), the total hospitalization cost decreased from39282.59yuan to36414.37yuan (P<0.01). There was an upward trend of Hospital seven days survival rate increasing from88.9%to96.7%(P<0.05)ConelusionThe "chronic obstructive pulmonary disease" and "respiratory failure" clinical pathways of integrated traditional chinese and western medicine can safely reduce the total length of hospitalization stay and the total hos-pitalization cost, improve7days survival rates during hospitalization.
