A Clinical Comparative Study Of Totally Extraperitoneal Prosthetic Verse Open Tention-free Inguinal Hernia Repair For The Syndrome Of Sinking Of Qi-deficiency Inguinal Hernia

Objective:To compare the clinical effect of laparoscopic totally extraperiton--eal prosthetic (TEP) with open tention-free inguinal hernia repair and to evaluate the feasible and economical methods for the syndrome of sink--ing of qi-deficiency inguinal hernia.Methods:A non-randomized concurrent control study was performed including134TEP(laparoscopic group)and130open tention-free inguinal hernia repair (open group) during Jan.2010and Jan.2012. We followed up the patients pos toperatively and compared the correlative parameters.Results:Compared with the open repair group, the laparoscopic group presented longer operating time[(68.2±19.7)min VS (61.4±16.6)min, P<0.05], higher cost in hospital [(12386.9±1181.8)元VS (11783.3±2794.6)元, P>0.05]. But the laparoscopic group had shorter postoperative hospital stay [(5.3+1.1)d vs (6.3±1.7)d, P<0.01], shorter time of return to normal activity[(12.6±3.0)h vs (21.0±4.2)h, P<0.01], less postoperative nalgesic requirement (6/134vs13/130, P<0.05). In the open group,2case had postoperative infection,4case had incision haematoma,3cases had scrotal swelling,7cases had urinary retention and3cases had chronic pain. While in the laparoscopic group, lease had postoperative infection,12cases had scrotal swelling,2cases had urinary retention. The total cases of complica-tions in the two groups were21and15,respectively. No significant difference was detected in complication between the two groups. All cases were followed up for3-24months with a median12months. In the open group,1case had recurred, no case recurred in the laparoscopic groups during that period. No significant difference was detected in recurrence between the two groups.Conelusion:TEP as one of the tention-free inguinal repair operations is feasible and safe for adult patients with inguinal hernia. It has clinical complementary with open tension-free hernia repair. We should select the correct choice of repair method according to the patient personal situation to gain the best clinical and cost-effective results.
