Clinical Application Of Totally Laparoscopic Extraperitoneal Inguinal Hernia Repair With Two Laparoscopes

Background and ObjectiveCompare with Open preperitoneal mesh repair(OPPM),Laparoscopic inguinal hernia repair(LIHR)can treat inguinal hernias with litter pain and fewer complications.Now,the main clinical operations of LIHR are transabdominal preperitoneal prosthes(TAPP)and totally extraperitoneal prosthes(TEP),TEP is more rational than TAPP because it’s operation in out of peritoneal cavity.But TEP is hard to operate,and often has many complications in both During operation and post operation.So our hospital try to use TEP with two laparoscopes treat inguinal hernia,studying the feasibility and the efficacy of TEP with two laparoscopes.MethodsThe clinical data of 51 patients with inguinal hernias underwent TEP with two laparoscopes from July 2015 to March 2016 in the third Hospital Affiliated to Anhui Medical University were analyzed retrospectively.Outcome measures are recorded: The operation time,The length of Postoperative hospital stay,Intraoperative blood loss,Postoperative complications(inguinal area seroma,inguinal region pain,postoperative urinary retention,infection);Followed-up by outpatient and telephone to observe whether inguinal region chronic pain,postoperative urinary retention,Recurrence is happened.The results of these date analyzed by the SPSS 18.0 statistical software.ResultAll the cases of TEP with two laparoscopes were successful.The operation time lasted 42 to 176 min with a mean of(74±27)min.The length of Postoperative hospital stay is 2 to 7 days with a mean of(4.0±1.1)days,and intraoperative blood loss was 5 to 30 ml with a mean of(12.8±5.9)ml.The percentage of postoperative complications was 11.8%(6/51),including inguinal area seroma which accounted for 2.0%(1/51),3.9%(2/51)of inguinal region pain,and 5.9%(3/51)of postoperative urinary retention,no infection were found.The follow-up period ranged from 3 to 11 months(median 7 months);no recurrence was observed during this period.ConclusionTEP with two laparoscopes can treat inguinal hernia precisely.It can also find compound inguinal hernia and occult inguinal hernia,apply to the treatment of inguinal hernia.
