The Initial Study Of The Regulatory Review And The Standard Of Practice For The Private Medical Institution In Luwan District

1. Research SignificancesAs an integral part of the medical service system, the private medical institutions are able to provide the diversified services to the society, meet the different levels of demand of the consumer groups and provide the community with more medical treatment options. However, with the rapid development of the private medical institutions, there appeared more problems. Due to the lack of overall long-term development plan, there have been issues with the inadequate management, non-standard practice and the poor legal knowledge which resulted in the medical argument and arbitration. The high rate of complaints force the medical inspection and administrative departments to focus on the private medical institutions. The question of how to regulate their practice has become the most important issue for the health department to face. Therefore, the research and analysis for establishing the regulatory standard, the evaluation criteria and the inspection for the private medical institutions become extremely important.2. Research ObjectivesThrough the retrospective study for the inspection and the management of the private medical institutions in China and abroad, also combining with the actual situations of the medical inspection and management in Luwan health department. To target the entire private medical institutions as the major study in Luwan District, analyzing their ways of practice in order to have the initial establishment of the evaluation and standard for the inspection and management regulations in LuWan District. All this is to improve the performance of health inspection department, establish a new model, further regulate the private medical institutions and optimize the Luwan District medical environment. 3. Research MethodsTargeting the entire private medical institutions as the study objects in Luwan District, analyzing their current situation, from government regulation, social supervision and self-management to establish the initial three latitudes practice supervision and management of the private medical institutions. According to the health-related laws and regulations, other methods developed through the literature search, the initial regulations and the operational methods have been established. Selection of 11 experts, convened an expert seminar to introduce the subject of the basic situation, the preferred assessment indicators and methods of operation. The use of Delphi Design of the questionnaire sought the second round experts’opinions which formed the private medical practice standards and regulatory review of the index weight for the Luwan District. Combining with the current situations of the private medical institutions in Luwan District, assess the classification within the 53 institutions in the district. Implementation will be made according to the actual assessment results.4.Research ResultsIn this study, the Delphi method was applied in this qualitative prediction indicators to evaluate and assess the value of the screening, establish the scientific references for evaluating the private medical institution supervision and management assessment. Assessment system is divided into two parts, including part of the annual assessment, the daily assessment. Part of the annual assessment, including government regulation, self-management, social supervision in three areas, medical institutions involved in the daily assessment of some of the staff, technology and equipment, quality and safety, prevention and control of infectious diseases, pathogenic microbiology laboratory, radiological health in seven areas. According to the results of the analysis from the experts, all the numbers of the active coefficiency. variation coefficiency, coordination coefficiency and the authority level have reached the standard in the Delphi method system.Therefore, the evaluation results of the consultation can be used as the references for the inspection criteria of the private medical institutions. From the different assessment levels in 2009 and 2010, personnel training, and self-correction of the medical institutions, compared with 2009 increased by 30.2% and 22.7%, the situation improved system was established by 13.3%.It is obvious that the assessment practice has changed the way of the operation of the private medical institutions, improved their self-awareness and the medical practice regulations as well as the establishment of the communication channels between the medical institutions and the administrative department. However, meanwhile there still exists the issues in regards to the illegal advertisement,35.7% of hospitals and outreach clinics exist non-standard,20.8% of private medical institutions, there is confusion in the management of medical instruments,32.1% of private medical institutions, medical personnel in the practice of non-standard. Therefore, the implementation of the assessment and management systems in the medical institutions is an important part and practice to regulate the medical practice and improve the medical safety. It is not only a new method to enhance the self-discipline for the private medical institutions, but also the new model for the health supervision departments to optimize the mode of supervision and to improve the regulatory efficiency.
