| Objective:To explore and compare the early clinical results efficacy of coblation nucleoplasty and conservative treatment for the treatment of discogenic back pain.Method:It was a retrospective contrast study to the result of surgical treatment and conservative treatment done in our hospital in 60 patients with discogenic back pain from 2008.12 to 2011.4.60 cases of discogenic back pain were divided into 2 groups:34 cases of treated by coblation nucleoplasty (D group,34 cases) and 26 cases of treated by conservative treatment(B group,26 cases). D group:21 males and 13 females, ranging between 19 and 60 years old. The average age was 43.7±5.3 years old, course of pain persisted ranging between 6 and 20 months, The average course of pain was 8.8+2.1 months.36 treated lumber discs in total, The treated discs located:1 in L3/4,20 in L4/5,11 in L5/S1,2 in both L4/5 andL5/Sl. B group:15 males and 11 females, ranging between 20 and 57 years old. The average age was 39.2+3.9 years old, course of pain persisted ranging between 6 and 16 months, The average course of pain was 8.3±1.9 months.27 treated lumber discs in total,The treated discs located:1 in L3/4,17 in L4/5,7 in L5/S1,1 in both L4/5 and L5/S1.The VAS,modified Stauffer-Coventry evaluation,styles of intervertebra disk, excellel-lent-good rate,patients’satisfaction rate and recurrence rate were contrast analyz-ed statistically postoperation after day sevenã€day fourteen and M3rd; Moreov-er, the recurrence rate and hospitalization wrere compared after M3rd and M6th.Result:The 2 groups has the same statistical result from age, sex,VAS,ODI and BMI before operation (P>0.05),it means comparability. The statistical difference between group D and B (before therapy vs after therapy) was significant in their time pointes (P<0.05), the same difference was found pre-therapy contrast to post-therapy in their respectively (P<0.05), the scores of group D was no difference between the March and day fourteen (P>0.05), but the scores of group B was difference between the March and day fourteen (P<0.05). The final follow-up clinical results efficacy of group (D,B),the rate of fineness were 91.1%and 65.3%, patients’satisfaction rate were 94.1%and 65.3%, Concerning chi-spuare test the result had a better statistical difference (P<0.01), group D did better than group B.2(5.8%) cases of group D and 9(34.6%) cases of group B were recurred,the statistical result was P<0.01, It meant that recurrence rate of group D did significant lower than recurrence rate of group B,9 cases of them were treated by coblation nucleoplasty and 2 cases of them were conservative treatment. The time of treatment of group (D,B) (X±S,天)were 5.8±1.7 and 14.5±2.6, Concerning t-test,it showed significant statistically different (P<0.01)Conclusion:coblation nucleoplasty method is better method for the treatme -nt of discogenic back pain. Because it has several advantages of easy operation ,more safe, minimal invasion,shorter time of recovery,least pain,excellent effect and less complication, these qualities have proven popular in patients. The successful rate of coblation nucleoplasty group precede that of the Conservative treatment group.As a new minimally invasive technique,nueleoplasty is an effective treatment for discogenic back pain in the short period,but middle and long treatment efficacy should be followed up and evaluated, the nucleoplasty technique should be used in selected cases before open surgery after conservative treatment has failed. |
