| Objective To explore the relationship between Th1/Th2balance and the clinical, pathological manifestations in IgA nephropathy.Methods40patients were enrolled who were pathologic diagno-sed primary IgA nephropathy in our department from Oct.2010to Jul.2011,excepting the secondary causes such as Lupus nephritis, Henoch-Schonlein Purpura nephritis, Hepatitis B Virus associated glomerulonephritis. None of the patients had been treated with glucocorticoid or(and) immunosuppressants in half a year. Peripheral blood samples from patients were obtained at the time of the renal biopsy. The clinical parameters including history, age, history of chronic tonsillitis and hypertension, serum Total Cholesterol (Tch), Triglycerides (TG), IgG, IgA, IgM, C3, C4, Creatinine Clearance (CCr) and urine red blood cells were collected. Lee classification was used for the grade of renal pathology, immune sediments including IgG, IgA, IgM, C3and C4were collected by semi-quantitative analysis. The percentage of Th1and Th2cells in the peripheral blood was detected by intracellular cytokine with flow cytometry, also calculated the ratio of Th1/Th2. The association of Thl and Th2cells with clinical and pathological parameters of IgAN patients was analyzed by Spearman rank or Pearson linear correlation analysis, and then Multiple linear correlation analysis.Results The percentage of Th1and Th2cells in CD3+CD8-T cells was (4.93±3.80)%and (1.77±0.90)%, the ratio of Th1/Th2was (2.75±1.72). The ratio of Thl/Th2is showing positive correlation with the value of IgG/IgA. The level of CCr declines with the values of Th1and Th1/Th2in IgAN patients. No significant correlation between Th2and clinical or pathological parameters was detected.Conclusions The Th1/Th2balance in IgA nephropathy patients with renal function decompositions is a Th2shift in peripheral blood, the cytokines of Th2may involve in the inflammatory response of IgAN and promote the development of renal impairment; The level of IgG/IgA in serum grows with the increasing of Th1/Th2in IgAN patients,the values of IgG/IgA can be an indirect reflection of Th1/Th2, which may be used for ascertaining the balance of Th1/Th2in IgAN. |
