Cannulated Screw Fixation For Femoral Neck Venus Compressed Three Hollow Screws In Treatment Of Femoral Neck Fractures

Ojective:To compare the clinical effects of Cannulated Screw Fixation for Femoral Neck (CSFN)and Threee compressed hollow screws In treatment of femoral neck fractures.Methods:The clinic data of67patients who had been treated for femoral neck fractures in our hospital from january2007to june2009were included for the present respective analysis. Divided into two groups based on patients voluntarily choose surgery,Of them,30cases were treated with CSFN and37with Compressed hollow screws. In the CSFN group,there are19men and11women.In the Compressed hollow screws group,there are19men and18women, Age31to71years and mean age:50.8years. According to the type Pauwells angle:In Pauwells angle≥50°,there are22cases treated with Threee compressed hollow screws and16cases of Cannulated Screw Fixation for Femoral Neck (CSFN);In Pauwells angle<50°,there are15cases treated with Threee compressed hollow screws and14cases of Cannulated Screw Fixation for Femoral Neck (CSFN). The two group were compared in duration of hospitalization. operation、intraoperative blood loss、infection、walking time、complication after operations、reduction quality、satisfaction、nonunion、femoral avascular、ratio of failure fixation and joint function (Harris’score).Results:The67patients received a mean follow-up of30to59months, an average of42months. From the perioperative indicators, There were no significant differences between the2groups in duration of hospitalization. infection、intraoperative blood loss、 walking time、 complication after operations (P>0.05). The operative time was average (31.1±9.7) min in Cannulated Screw Fixation for Femoral Neck and average (40.4±12.7) min in Threee compressed hollow screws; The average operative time in Cannulated Screw Fixation for Femoral Neck is longer than Threee compressed hollow screws; There were significant differences between the2groups in duration of operation. From the postoperative and long-term complications assessment, There were no significant differences between the2groups in reduction quality、satisfaction、 nonunion、and femoral avascular.(p>0.05) According to ratio of failure fixation in the pauwels≥50°,there were not appeared to retreat nails, broken nail, screw cutting the femoral head and other failure cases in Cannulated Screw Fixation for Femoral Neck. While there were6cases of failure fixation in Threee compressed hollow screws; Two cases appear retreat nails, two cases of screw cutting the femoral head, the two cases show that the screw is not completely into the femoral. There were significant differences between the2groups in ratio of failure fixation in the pauwels≥50°According to ratio of failure fixation in the Pauwells<50°, One cases of screw loosening in Cannulated Screw Fixation for Femoral Neck group and One cases of screw reback Threee compressed hollow screws group. There were no significant differences between the2groups in ratio of failure fixation in the Pauwells<50°.The patients who suffer in screw loose, retreat nails and cutting the femoral head is more than65years. Final follow-up, There were no significant differences between the2groups in the joint function of6months after operations with outstanding rate and good rate (p>0.05),while There were significant differences between the2groups in the joint function of30months after operations (p>0.05)Conclusion:Cannulated Screw Fixation for Femoral Neck has minimally invasive, simple, fixed indeed in treatment of femoral neck fractures.Compared with the traditional three hollow screw system group, Cannulated Screw Fixation for Femoral Neck has less operative time, less fixation failure rate, and a higher hip function score in postoperative average of30months. It is a good effect in treatment of femoral neck fractures,especially for whose Pauwells≥50°and age>65years.
