| [Objective]This study aims to present an improved technique of harvesting the maximal amount of the auricular cartilage while preserving auricular framework. Also discussed is the analysis of Chinese nasal tip morphological characteristics and the auricular cartilage utilization and evaluation in nasal tip refinement.[Methods](1)Analysis the anatomical basis and support structures of the auricula through the anatomical study.Find out the key regions that maintain the framework of the auricula. Make it clear that which region of the cartilage can be harvest safely and maximal.Choose the best surgical incision position.(2) Observation and analysis were made in clinic of the Chinese tip characteristics. Auricular cartilage harvest and nasal tip refinement operations performed and evaluated.[Results]The structure which maintain the auricular framework including helix, crus of helix, antihelix, superior and inferior crus of the antihelix,tragus,antitragus. The inferior crus of the antihelix, the root of the helix and the area where the concha cavum transitions into the posterior-inferior margin of the external auditory canal are very important with the function of maintaining the three-dimensional poison of the ear. The concha cymba,concha cavum and the hunch between them can be harvest safely. The ideal incision is in the auriculocephalic sulcus posterior of the ear. Patients were divided into two groups according to the characteristics of the nasal tip (1) underprojection with normal tip rotation and (2)underprojection with superior tip rotation. The procedure was used in clinic and got a satisfied result.[Conclusions](1) Graft of auricular cartilage is adequate for tip refinement. The concha cymba, concha cavum and the hunch between them can be harvest safely.(2) Bulbous nasal tip, short columellar, superiorly rotated and underprojected nasal tip are common deformities in Chinese people.(3)Nasal tip refinement using autogenous auricular cartilage is reliable and advisable. |
