Objective:A randomized controlled trial of the clinical effect of acupuncture plus rehabilitation training in treating early-stage shoulder-hand syndrome due to ischemic stroke. To determine therapeutic effects by pain measurement and movement function evaluation and to provide a new combined modality therapy.Methods:1. The subjects were recruited from the No.2Ward of Acupuncture-moxibustion Department, No.2Hospital Affiliated to Heilongjiang University of Traditional Chinese Medicine. All60cases of early-stage shoulder-hand syndrome due to ischemic stroke were randomized into an observation group and a control group according to the internationally recognized random alphabet table,30in each group.2. The two groups both received routine treatment according to the Guideline for Prevention and Treatment of Cerebrovascular Diseases (2005). plus scalp acupuncture at the Anterior Oblique Line of Vertex-Temporal (MS6). The scalp acupoints were located based on the Standardized Manipulations of Acupuncture and Moxihustion-Part2:Scalp Acupuncture (GB/T21709.2-2008). The needles were retained for30min after qi arrival, and the needles were manipulated every10min during retaining. The observation group received needling distal points plus exercise, the needles were retained for20min. The patients were asked to bend, stretch, abduct, adduct. externally rotate, and internally rotate the upper limb during needle retaining, all within their maximum endurance. Both groups receive rehabilitation under professional guidance by adopting Bobath therapy for20min each time. The treatment was given once a day in the two groups,6times as a treatment course, with a day interval between each two courses.3. Observation of therapeutic efficacy using the visual analogous scale (VAS). Fugl-Meyer assessment scale (FMA). edema degree of hands and shoulder activity degree. 4. All data were dealt with SPSS18.0statistical software. The enumeration data were compared by using Chi-square test, and the measurement data were by t-test. Results:Total effective rate was93.33%in treatment group while70.00%in control group with significant differences (P<0.05). Intra-group comparison of VAS, FMA, edema degree of hands and shoulder activity degree had statistical (P<0.05or P<0.01). Both therapies were effective while the result of acupuncture plus rehabilitation training in treating early-stage shoulder-hand syndrome due to ischemic stroke is superior to rehabilitation training alone.Conclusions:1. The result of needling distal points plus exercise combining with rehabilitation training in treating early-stage shoulder-hand syndrome due to ischemic stroke is superior to rehabilitation training alone.2. Needling distal points plus exercise combining with rehabilitation training can improve VAS scores, upper limb movement function, edema degree of hands and shoulder activity degree etc.
