Background:Plaster in Traditional Chinese Medicine(TCM),Plastering the aeffcted part directly, working through the skin, can activate blood circulation, remove blood stasis, subdue swelling and alleviate pain.Shang Ke XiaoYan Gao(SKXYG)of Jiang Su Province Hospital of TCM as a Plaster has been used to treat Knee Joint Osteoarthritis(KJOA)excellently for several decades.Shang ke xiao yan gao can obviously help relieving synovitis,alleviating the swell and pain of knee ostearthritis.But its action mechanism is still unclear. We observed the synovial cytokine level and pathology changes in Papain induced rabbit knee osteoarthritis after the therapy of Shangkexiaoyangao to.Objective:To observe the influence of Shangkexiaoyangao on synovial Nitric Oxid(NO) interleukin-1(IL-1), leukotriene(LTB4),pathology in Papain induced rabbit knee osteoarthritis and explore the action mechanism.Methods:Papain solution was injected into the right knee joint cavity of15New Zealand white rabbits on the1st,4th and7th days to induce knee osteoarthritis in rabbits. About0.5ml synovium near the fatty pad of each rabbit knee joint was acquired at the2nd,4th and6th weeks after first injection.There are4rabbits in every time point.The synovium of the4normal rabbit knee joints was acquired at the2nd week simultaneously.The general and histopathologic changes of rabbit knee synovium were examined, to make sure that the synovium inflammatory reaction of knee joint in rabbits reach its maximum effect in the2nd week after the first injection of papain.The18Papain induced rabbit knee osteoarthritis modles were divideded into blank control group, Shangkexiaoyangao group and diclofenac diethylamine group randomly.6rabbits in each group. The latter two groups were treated with Shangkexiaoyangao and diclofenac diethylamine separately while there is no therapy on blank group. The rabbits were killed at the11th day after therapy and about0.3ml synovium near the fatty pad of each rabbit knee joint was acquired. The NO,IL-1,LTB4of knee synovium were examined by ELISA method.The histopathologic changes and immunohistochemistry of knee synovium were examined.Then observe the inflammation, necrosis, fibroplasia and mucinous degeneration of knee synovium by histopathology.Results:The NO level of blank control group is high than Shangkexiaoyangao group (P<0.05).The IL-1level of blank control group is high than Shangkexiaoyangao group (P<0.05). The LT level of blank control group is high than both Shangkexiaoyangao group and diclofenac diethylamine group (P<0.05).There is no difference with theNO,IL-1,LTB4level between Shangkexiaoyangao group and diclofenac diethylamine group (P>0.05). The inflammatory reaction of blank control group on synovium is more severe than Shangkexiaoyangao group and diclofenac diethylamine group by histopathologic.Conclusion The results indicate that Shangkexiaoyangao can relieve the synovitis of knee osteoarthritis by decreasing the inflammatory factors. The therapeutic effect of Shangkexiaoyangao is better than diclofenac diethylamine.
