Therapeutic Effect Of Arthroscopy Combined With Taohong Siwu Decoction And Wuling Powder On Knee Osteoarthritis With Synovitis

Objective:To explore the efficacy and safety of arthroscopy combined with Taohong Siwu Decoction and Wuling Powder in the treatment of knee osteoarthritis with synovitis,and provide guidence for clinical treatment of knee osteoarthritis with synovitis.Method:Selected from January 2017 to June 2018 in the Department of Orthopedics and Traumatology of Hubei Hospital of Traditional Chinese Medicine,patients with knee osteoarthritis with synovitis were randomly divided into treatment group(34 cases)and control group(34 cases),68 cases in total.The treatment group was treated with knee arthroscopic debridement combined with Taohong Siwu Decoction and Wuling Powder,it were taken on the first day after operation,decoction one dose per day,and it were taken warm after meals twice in the morning and evening for 28 consecutive days.While the control group was treated with arthroscopic debridement combined with synovitis granules,it also were taken on the first day after operation,one bag each time,and three times a day for28 consecutive days.Both groups of patients started knee joint function exercise on the first day after operation under the guidance of doctors.Compared the clinical data,knee pain,knee swelling,knee function and main biochemical indicators between the treatment group and the control group before and after operation.The pain of knee joint was evaluated by VAS score before operation,on the first day after operation,on the seventh day after operation,on the fourteenth day after operation and on the twenty-eighth day after operation.The knee function was evaluated by Lysholm score before operation,on the seventh day after operation,on the fourteenth day after operation and on the twenty-eighth day after operation.The knee joint swelling was assessed before operation,on the seventh day after operation,on the fourteenth day after operation and on the twenty-eighth day after operation according to the knee joint circumference,and all the patients were followed up 6 months after treatment,then compare the curative effect between the two groups.Results:A total of 67 patients were effectively returned visit when 6 months after treatment.The wounds healed well in both groups,and the sutures were removed on time.There were no obvious discomfort and adverse reactions after taking medicine in both groups,and no obvious complications after operation.There were no significant abnormal changes in blood,urine,liver and kidney function and electrocardiogram before and after treatment.The results of statistical analysis is showed that:Statistical analysis showed that there was no significant difference in age,gender,location of disease and course of disease between the two groups,P>0.05,is comparable.According to preoperative VAS score,Lysholm score and knee circumference of the two groups were statistically analyzed,P>0.05,with no significant difference,is comparable.According to the VAS scores of the two groups on the 7th,14 th and 28 th day after operation were statistically analyzed,the treatment group was better than the control group,and P<0.05.The difference was statistically significant.It can be considered that the pain relief of the treatment group was better than the control group.The Lysholm scores of the two groups on the 7th,14 th and 28 th day after operation were statistically analyzed,the treatment group was better than the control group,and P<0.05.The difference was statistically significant.It can be considered that the improvement of knee joint function in the treatment group was better than the control group.The knee circumference of the two groups on the 7th day,the 14 th day and the 28 th day after operation was statistically analyzed,the treatment group was smaller than the control group,and P<0.05.The difference was statistically significant.It can be considered that the treatment group was better than the control group in eliminating knee swelling.Follow-up 6 months after treatment,the total effective rate of the treatment group was 97.06%,and the control group was93.94%.The statistical analysis showed that P > 0.05,there was no significant difference between the two groups,it can be considered that there is no significant difference in long-term efficacy between the two groups.Conclusion:Arthroscopy combined with synovitis granule or Taohong Siwu Decoction and Wuling Powder can effectively treat knee osteoarthritis with synovitis.However,the short-term effects of patients who taking Taohong Siwu Decoction and Wuling Powder on relieving pain symptoms,eliminating knee swelling and functional recovery after arthroscopy are better than those patients who taking synovitis granules after arthroscopy.Follow-up 6 months after treatment,there was no significant difference in the total effective rate between the two groups,so it can be considered that the long-term effect was similar.There were no obvious adverse reactions and side effects in both groups.Therefore,arthroscopy combined with Taohong Siwu Decoction and Wuling Powder is effective and safe in the treatment of knee osteoarthritis with synovitis,which is worthy of further clinical research and promotion.
