Assessment Of Left Ventricular Wall Motion State And Diastolic Function Of Coronary Artery Disease By Real-time Three-dimensional Echocardiography

Objective:To evaluate the diastolic function and the displacement of the left ventricle (LV)with coronary artery stenosis by real time three-dimensional echocardiography(RT-3DE).Methods:One hundred and ten patients were examined by RT-3DE. Among them,75patients with coronary heart disease (CHD group), which were diagnosed with onecoronary artery lesions by stenosis greater than50%by PCA; and35patients withoutCHD (control group). The displacement and mean filling rate (MFR) of the leftventricular region with coronary artery stenosis were calculated by Q-lab software.Then, the correlations between displacement, MFR and the degree of coronary arterystenosis were evaluated. While, the correlations between the displacement and theMFR was evaluated too.Results:①Illustration of the bull eye(IBE) showed: the displacement of the ischemicregion was decreased even negative and contradictory movement in CHD group,correspond with control group②Compared with control group, MFR and displacement were decreased in CHDgroup, whereas left ventricle ejection fraction (LVEF) was decreased and enddiastolic volume(ESV) was increased. There were significant differences betweentwo groups (P<0.01).③The displacement in left ventricular regional wall with the correspondingcoronary artery stenosis (left anterior descending,LAD;left circumflex,LCX; rightcoronary artery,RCA) in the subgroup of stenosis (50%-70%) and control group were4.85±0.87mm vs6.67±1.41mm,4.49±0.94mm vs8.27±2.23mm,5.48±1.39mm vs6.63±1.67mm, respectively; and MFR were6.05±1.88ml/s vs8.15±3.20ml/s,5.56±1.80ml/s vs11.26±3.28ml/s,7.77±2.76ml/s vs8.12±2.84ml/s, respectively. There were significant differences in two groups(P<0.01).④The displacement of the left ventricular regional wall with the correspondingcoronary artery stenosis (LAD, LCX, RCA) in the subgroup of stenosis (more than70%) and control group were2.39±1.03mm vs6.67±1.41mm,2.28±1.11mm vs8.27±2.23mm,2.27±1.05mm vs6.63±1.67mm, respectively, and MFR were3.75±1.06ml/s vs8.15±3.20ml/s,4.08±1.21ml/s vs11.26±3.28ml/s,3.49±1.03ml/svs8.12±2.84ml/s, respectively. There were significant differences in two groups(P<0.01).⑤In CHD group, the displacement of the left ventricular regional wall ofcorresponding coronary artery stenosis and MFR negative correlated highly with thedegree of arterial stenosis(r=-0.816,P<0.01and r=-0.708,P<0.01). In CHD group,the displacement of the left ventricular regional wall of corresponding coronary arterystenosis highly correlated with the MFR in CHD group (r=0.678,P<0.01).Conclusions:①RT-3DE ’s IBE can be used to evaluate left ventricular wall motion.②The displacement of the corresponding regions of stenotic artery in CHDgroup was significantly lower than corresponding control group, and wassignificantly negative correlation with the degree of arterial stenosis.③The MFR of the corresponding regions of stenotic artery in CHD group wassignificantly lower than control group, and was significantly negative correlation withthe degree of arterial stenosis.④The displacement of the corresponding regions of stenotic artery wassignificantly positive correlation with the MFR in CHD group.
