Objective:Coronary heart disease(coronary heart diseases,CHD)patients with left ventricular function is divided into systolic function and diastolic function,some patients may have early diastolic function is impaired,even in the presence of systolic function loss before.The causes of impaired left ventricular diastolic function is numerous,including myocardial ischemia is an important factor.At present,the checking method of left ventricular diastolic function is numerous,real-time three-dimensional echocardiography(real time three-dimensional echocardiography,RT-3 DE)can check accurate noninvasive repeatable reflect left ventricular diastolic function,make for a more in-depth research to explore clinical workers.We intended to use RT 3 DE check,combines the technique of electrocardiogram(ecg)and 2 DE,explore the RT-3 patients with coronary artery disease DE parameters,electrocardiogram(ecg)and the relationship between the left ventricular diastolic function.Methods:19 in 38 patients with CHD angina pectoris and 19 myocardial infarction,eliminate high blood pressure,diabetes,previous treatment of patients with a history of myocardial infarction medical history and revascularization,and 18 healthy subjects(control group)RT-3 DE inspections are voluntary,take QLAB7.0 image analysis software packages,all images are recorded CD save,each patient with 3 d probe-time curve for left ventricular volume(volume-time curve,n C),left ventricular end-diastolic volume software automatic measurement data(left ventricular end-diastolic volume,LVEDV),left ventricular end systolic volume(left ventricular end-systolic volume,LVESV),heart rate,heart rate,HR),left ventricular ejection fraction(left ventricular ejection fraction,LVEF),in the end systolic to calculate EDV early when mitral valve open image/EDV %,observing the overall and each segment VTC group form to calculate the average filling rate(mean filling rate,MFR).Three groups all do automatic 12 lead ecg examination,calculate the ptfv1 and QT dispersion degree(QTd),applied statistics analysis of patients with coronary heart disease(CHD)of real-time three-dimensional echocardiography parameters and ptfv1 and the correlation of QTd.Results:1.In a routine check of 2DE,a group of patients with myocardial infarction and angina pectoris group compared with control group,the E/E ’ratio,LVEDV,LVESV increase;Peak ratio and E/A,E ’decline.Myocardial infarction group the angina pectoris group and the control group there is a decrease in the LVEF.2.In RT-3 DE examination,early myocardial infarction group compared with control group EDV,LVEDV,LVESV/EDV % increase,and LVEF and the decrease of the MFR.Myocardial infarction group compared with angina pectoris group,LVEF,MFR decreased.Angina pectoris group compared with control group between EDV early/EDV %,LVEDV,LVESV have increased.3.2DE and RT-3DE in measuring EDV value(P < 0.05,there are differences,there is statistical significance,while the ESV,LVEF value is no.4.In ECG examination,myocardial infarction and angina pectoris group compared with control group,QTd increase;Increasing Ptfv1 negative.A group of patients with myocardial infarction and angina pectoris group than,increasing Ptfv1 negative.5.Electrocardiogram Ptfv1 value positively correlated with MFR value,and negatively correlated with EDV early/EDV% values;QTd value,on the other hand,negative correlation with MFR value,and EDV early/EDV % values were positively correlated.EDV early/EDV % evaluate left ventricular diastolic function correlation is better.Conclusion:(1)real-time three-dimensional echocardiography can accurately measure the LVEF,left ventricular volume,EDV early/EDV%,MFR,thus effectively quantitative evaluation of the changes of left ventricular diastolic function in patients with coronary heart disease(CHD).(2)the electrocardiogram Ptfv1,QTd provide the basis for the early detection of coronary heart disease patients abnormal cardiac function.(3)both in the diagnosis of correlation,the joint assessment of coronary heart disease heart function can improve the sensitivity and specific degrees. |
