Objective: To observe the ovarian shape,blood flow parameters ofpatients with PCOS and patients with POF using transvaginal colorDoppler ultrasound(TVCD), and to study the application value of thesesparameters in diagnosis and treatment of PCOS and POF.Methods: We observed168patients by detecting ovarian shape andblood flow parameters using TVCD and measuring endocrine hormone.But in the end33PCOS patients,18POF patients and31normalovulation patients (as control group) fitted the standards and contributeddata to statistical analysis of our study. Ovarian shape,blood flowparameters (PSV,EDV,RI andPI)were observed at the time of basalstate/basal state-like and at the time of preovulation/preovulation–like{level of estrogen reaching250-350pg/ml;endometrium≥10mm;typeA;diameter of dominant follicle18-20mm (control group/PCOS groupafter ovulation induction)}. Data analysis was performed by t test.Results:1.Basal state/basal state-like: Number of antral follicles:PCOS group(16.23±3.46)is larger thancontrol group(7.33±2.57), there’s statistical difference (P<0.05); POFgroup (1.05±0.99)is lower than control group(7.33±2.57), there’sstatistical difference (P<0.05); PCOS group(16.23±3.46) is larger thanPOF group(1.05±0.99), there’s absolutely statistical difference (P<0.001).Ovarian volume:PCOS group(13.88±3.65)ml is bigger than controlgroup(6.67±2.45)ml, there’s significant difference(P<0.01); POFgroup(1.02±0.79)ml is smaller than control group(6.67±2.45)ml, there’ssignificant difference(P<0.01); PCOS group (13.88±3.65)mlis biggerthan POF group(1.02±0.79)ml, there’s absolutely statistical difference(P<0.001).Blood flow parameters:PCOS group PSV(22.23±5.12)cm/s,EDV(7.43±3.23)cm/s, RI(0.66±0.04), PI (0.97±0.15); POF group PSV(17.43±3.57)cm/s,EDV(5.39±1.04)cm/s,RI(0.73±0.16),PI(1.27±1.32);control group PSV(21.08±4.53)cm/s,EDV(6.75±4.52)cm/s, R(I0.68±0.05),P(I1.01±0.13)。PCOS group compared with controlgroup,there’s no statistical difference (P>0.05); POF compared withcontrol group, there’s statistical difference (P<0.05); PCOS groupcompared with POF group, there’s statistical difference (P<0.05).2.Preovulation/preovulation-like: PCOS group PSV(26.82±5.16)cm/s,EDV(10.84±3.68)cm/s,RI(0.58±0.04),PI(0.90±0.15);POF group PSV(23.64±3.27)cm/s,EDV(7.87±2.24)cm/s,RI (0.67±0.16),PI(1.02±1.22);control group PSV(25.45±4.63)cm/s,EDV(9.75±4.72)cm/s,RI(0.60±0.05),PI(0.97±0.13)。There is nostatistical difference among them(P>0.05).3.PSV(23.64±3.27)cm/s and EDV(7.87±2.24)cm/s of ovary arteryof POF group post-treatment are higher than POF group without treatmentPSV(17.43±3.57)cm/s and EDV(5.39±1.04)cm/s, there’s statisticaldifference (P<0.05); R(I0.67±0.16)and P(I1.02±1.22)of ovarian arteryare lower than POF group without treatment RI(0.73±0.16)and PI(1.27±1.32), there’s statistical difference (P<0.05).Conclusion:1.Basal state/basal state-like: Blood flow of ovarian stroma of PCOSPatients has no difference compared with control group.Patients withPOF have lower blood flow of ovarian stroma than control group andPCOS group.The values of PSV and EDV reduce,the values of RI and PIrise,and it is in a state of low-speed and high resistance.2. Preovulation/preovulation-like: blood flow of ovarian stromaamong PCOS group,POF group and control group has no significantdifference.3.Blood supply of ovary in POF patients after hormone replacementtreatment will be improved.4.Two-dimension ultrasound is more accurate,intuitive and simple indiagnosis of PCOS and POF,while color Doppler ultrasound is useful for assessment of treatment effects.
