Objective To observe the different surgical treatment in simple hepatic cyst, and compare the clinical advantages and shortages to choose the best surgical method for this disease.Methods Retrospective analysis and composing data with the SPSS18.0of71cases of simple hepatic cysts in our hospital from January2009to September2011.20cases were male and51were female, age from40to82years old, and the average age of59.6±7.6years. There are three groups:1) B super-mediated puncture and drainage hardening treatment group;2) open surgery group;3) laparoscopic cyst fenestration group.71cases were all made the diagnosis of simple liver cyst, and parasitic hepatic cyst, traumatic cyst, inflammatory cyst and neoplastic cyst and other non simple liver cyst disease were removed by preoperative B ultrasound or dynamic CT scan with the clinical history.41cases were followed up from3to37months.Results All of71cases were released from their pre-operative symptoms. The operative time (34.8±9.4min) and hospital stayed (3.5±1.6d) of B ultrasound mediated puncture and drainage sclerotherapy group were the shortest in the laparoscopic and open surgery groups, of which operative time (71.2±36.6min), length of stay (5.2±2.1d) and the operation time (125.7±38.4min), length of stay (12.5±4.1d), respectively. In laparoscopic group, the drainage time (3.5±1.6d), bleeding (20.7±12.3ml) were lower than in the laparotomy group which of drainage time (7.5±2.8d), bleeding (60.5±20.1ml). Three cases of complications were occurred in laparotomy group after.1case was found symptom recurrence in the advanced B ultrasonic-mediated percutaneous drainage combined with dehydrated alcohol sclerotherapy (5.5%),2cases were discovered symptom in laparoscopic surgery(11.3%). And,3cases were recurred in the laparotomy(8.3%),5cases with complications in all of patients.Conclusions Ultrasonic-mediated percutaneous drainage combined with dehydrated alcohol sclerotherapy is particularly suitable for the simple hepatic single cysts. In the invasive treatment of simple liver cysts, laparoscopic surgery is the first choice, and laparotomy is more suitable for multiple cysts, and laparoscopic drainage or percutaneous drainage combined with sclerotherapy could not be completed.
