| Objective:1.To observe the changes of intestinal function after burn.2.Toobserve the influence of Sijunzi decoction on intestinal nutritional status,immunity and motor function postburn. Methods:1.The preparation ofTraditional Chinese Herbal Medicine Pilose Asiabell Root12g, Indian Buead12g, Largehead Atractylodes Rhizome12g, Liquorice Root12g, Rhubarb6g,one agent per day, detecting25minute in water.2. The selection and groupingof cases Internalize standard:①admitted to hospital in8hour after hurt,②the burn total area between30-50percent of total body surface area, withoutsevere shock and inhaling injury,③18to55years old, either male or female.To exclude:①severely associated injury,②diabetes mellitus, pregnancy,breast-feeding, severe heart, liver and kidney dysfunction,③cancer disease inthe recent5year,④allergy to the drug component researched. Burned patientswere randomly divided into control group and treatment group according torandom number method.3.Procedure There was no difference betweenconventional and wound therapy, meanwhile, the patients in treatment grouptook the decoction orally100ml twice after meal per day from the day after hurt,until the10th day. The control group took physiological saline orally with thesame methods and quantity mentioned above. Dynamically observe andcompare the digestive function of each group (e.g. diet, stools, abdominaldistension, nausea, vomiting condition and so on). The venous blood wassampled from patients before taking drugs and1,4,7,10d after meditation, and the content of serum ALB, PA, IgA, IgG, IgM, CD4+, CD8+, NO and motilinwere detected, then calculating the CD4+/CD8+ratio.4. Statistical method Usingthe statistical software of SPSS17data analysis, all data were measured asmean±standard deviation, and rate to express enumeration data. The analyticmethod is mainly analysis of variance and X2test, P<0.05showed significantstatistical difference. Results:1. Digestive function There were16(64%) and10(40%) with effect in treatment group and control group respectively, and thedifference was statistically significant(p<0.05,similarly hereinafter).2.Nutritional status The ALB and PA of control group the treatment groupdecreased gradually, significantly, by20.23%and52.95%till the10thday. TheALB increased significantly and higher than the baseline4d after meditation inthe treatment group, and PA heightened in the7thday too. On the10thday ALBand PA recovered to the normal range, and significantly higher than the statebefore meditation. Compared with the two groups, treatment ALB and PAincreased obviously than the control group4days after meditation, withstatistical difference.3. Immunoglobulin The serum IgA in control group wassignificantly decreased7d after meditation, and IgG, IgM declined continuouslyin the process of therapy. The IgA in treatment group4d after meditation wasapparently higher than taking drugs before, IgG, IgM increased on the7thday,with significant difference. Compared with the two groups, the treatment group,IgA IgM higher than the control group on the4thday, IgG also increasedsignificantly on the7thday, and the difference between them was statisticallysignificant.4. Peripheral blood T lymphocyte subgroup The number of CD4+ and CD8+cells reduced in control group after the injury, CD4+/CD8+ratiofluctuated. After meditation the CD4+and CD8increased on the4thday,CD4+/CD8+reduced. There was a significant difference between the two groups.5. Gastrointestinal motor function The level of serum motilin in the controlgroup did not change significantly, while the treated group4d was significantlyhigher than that of before medication. Compared with the control group, thedifference was statistically significant.6. Biological active medium After theinjury the NO content in serum of two groups was decreased, only in4d,7d oftreatment group the difference was statistically significant compared withcontrol group. Conclusion: Sijunzi Decoction can improve the digestivefunction of burn patients by raising serum ALB and PA content, and promotethe recovery of humoral immune function by regulating T lymphocyte subsetsratio. In addition, it may enhance gastrointestinal movement by decreasing thecontent of NO in serum, thus plays a positive role on postburn intestinalfunction improvement, and it is worthy of clinical application. |
