Objective:1. To analyze the patients’clinical characteristics, and find out the related factors forthe sudden sensorineural hearing loss (sudden deafness).2. To analyze the related factors between the complication and its severity for thesudden deafness, and investigate its pathogenesis.3. Explore the clinical significance from the blood tests and various factors of suddendeafness among different severity patients.4. Comprehensively analyze the related causes and factors of sudden deafness, sothat these could guide clinical treatment and improve the cure rates.Methods:1. From December2010to December2012, selected176cases who diagnosed ofsudden deafness as the experimental group, while random selected60cases from thenormal physical examination as the control group, then collect their medical historyinformation in detail, in Yan’an University Affiliated Hospital.2. As the blood samples indicator detection,we draw off the peripheral blood samplesfrom the fasting patients with sudden deafness on the next morning after the patientshad been hospitalized,and then sent the blood samples to the laboratory forexamination.3. To analyze the collected data by SPSS17.0. Univariate analysis: the mean of twogroups were compared by t/t’ test. The constitute ratio of two groups was compared by Chi-square test. Multivariate analysis: use the binary Logistic regression model toanalyze the relationship of multiple factors and the onset of sudden deafness.Analyze the relationship of multiple factors and the severity of sudden hearing lossby four models on the binary Logistic regression analysis.Results:1. Hematological indexes of patients with sudden deafness: blood viscosity increased(whole blood viscosity is high cut and low cut), clotting activity enhanceed(â…§:Cã€VWFã€PLTã€AT-â…¢ã€PLGã€Fg and α2-AP),serum biochemical (TP,Alb,FBG,LP(a),apo-B,TC,TG,VLDL and K+) and the level of white blood cells (WBC andNEUT)were compared with the control group. There was a significant deviationbetween the two groups (p<0.05).2. Analyzed by the binary Logistic regression model,from176cases we got a resultthat the odds ratios of sleep duration(<7hours), hypertension, hyperglycemia,diabetes, cerebral blood supply insufficiency, cold history, mental and psychologicaldisorder, mobile phones and other electronic radiation, hyperlipidemia and bloodviscosity increased were above1(OR>1), which was significantly relationship withsudden hearing loss.3. Using four models on the binary Logistic regression model to analyze suddenhearing loss level that it had a certain relationship with sleep duration(<7hours), coldhistory, hypertension, hyperglycemia, diabetes, hyperlipidemia, cerebral bloodsupply insufficiency, blood viscosity increased, mental and psychological disorder,hypokalemia, and Anemia(p<0.05). Elevated blood viscosity acted as an importantrole in the mild and moderate hearing loss, and is one of the most important riskfactors of sudden hearing loss. Sleep duration and Cold history played an importantrole in the mild, moderate, severe and very severe hearing loss. Diabetes is closelyrelated to the severe and very severe sudden sensorineural hearing loss. They areclosely related to the very severe sudden sensorineural hearing loss withhyperglycemia, hyperlipidemia, cerebral blood supply insufficiency, and hypokalemia.Conclusion:1. Sleep duration(<7hours), hypertension, hyperglycemia, diabetes, cerebral bloodsupply insufficiency, cold history, mental and psychological disorder, mobile phonesand other electronic radiation, hyperlipidemia and blood viscosity increased are therisk factors of sudden hearing loss.2. Sudden hearing loss level had a certain relationship with sleep duration(<7hours),cold history, hypertension, hyperglycemia, diabetes, hyperlipidemia, cerebral bloodsupply insufficiency, blood viscosity increased, mental and psychological disorder,hypokalemia, and Anemia(p<0.05).3. The occurrence of sudden hearing loss was related to increased blood viscosity,enhanced clotting activity, elevated blood lipids and white blood cell levels changes. |
