Background and Objective:Studies have shown that ischemic stroke accounts for about80%of the total stroke events, so as to ischemic stroke is the most common type of cerebrovascular disease. The incidence of acute cerebral infarction in China is still an increasing trend of current high level, which has become one of the common diseases. Atherosclerosis is one of the basic cause of cerebral infraction especially carotid atherosclerosis, accompanied by the intensively close relationship between dyslipidemia and atherosclerosis. Recent studies show that relating to cardiovascular and cerebrovascular diseases, apolipoprotein A1, apolipoprotein B and ApoB/ApoAl are probably risk factors. But some other researches suggested that ApoAl was a moderate degree risk factor while ApoB has no apparent correlation. Comparing the different levels of apolipoproteins and the lipid between cerebral infarction group and the control group, as well as that of carotid stenosis of different degrees affected by acute cerebral infarction patients, the aim was to discuss the correlation between the stenosis degree and the level of apolipoproteins, so as to find the affection in cerebral infarction related to apolipoproteins, and the risk factors in cerebral infarction and carotid artery atherosclerosis.Methods:1、131acute cerebral infarction patients were divided into four groups according to the degrees of carotid atherosclerosis and38healthy people were included as control group. The serum levels of TCH、TG、LDLC、HDLC、ApoA1、ApoB were detected.2、According to the carotid artery stenosis degree, cerebral infraction group was divided in to:①No carotid artery stenosis or stenosis rate of1%-15%;②Stenosis rate of16%-49%;③Stenosis rate of50%-79%;④Stenosis rate of80%-99%or complete occlusion.3、Compare the apolipoproteins and the lipid levels between cerebral infarction group and the control group, and the different degrees of carotid stenosis among the four groups which were acute cerebral infarction patients; explore correlation between the degree of stenosis and the apolipoproteins; stepwise regression analysis was used to analyze possible risk factors for cerebral infarction and vascular stenosis.Results:Compared with the control group, the serum ApoB, ApoB/A1levels of cerebral infarction patients were significantly higher (P<0.001) while serum ApoAl, HDLC were significantly lower (P<0.001). There were significant differences in serum ApoB and ApoB/A1among the four groups (P<0.001) but not in ApoAl. ApoB/Al and ApoB (P<0.001) was positively correlated with the degree of carotid stenosis while ApoAl was negatively correlated. Multiple regression analysis showed that serum ApoB/Al levels was risk factors for carotid stenosis(P<0.001).Conclusion:1.The serum ApoB/ApoAl, ApoB levels of patients with acute cerebral infarction may be a risk factor for cerebral infarction; while ApoAl level may be a protective factor for cerebral infarction patients.2.The serum ApoB/ApoAl level was closely related with carotid atherosclerosis and the degree of carotid stenosis.
