Objective:To observe the influence of penehyclidine hydrochioride (Penehyclidine)on heart rate variability (HRV) underlying gynecological hysteroscopicsurgery under the intravenous anesthesia.Method:Selected in October2011to may2012at the First Hospital of JilinUniversity gynecological elective laparoscopic surgery were60cases. ASAphysical status I~II level, age30to50years(mean45±1.47years),weight(50~70)kg, preoperative cardiovascular system and central nervous systemabnormalities, diabetes, and the use of atropine or Changtuoning to tabooPatients were excluded from the study. Control group (group I; n=20),Experimental group penehyclidine group (group II; n=20) and Control groupsaddle block group (group III; n=20). Preoperative fasting8h into theoperating room after their electrocardiogram (ECG), heart rate (HR),oxygensaturation (SpO2), blood pressure (BP) monitoring, open venous access.Group I underwent intravenous anesthesia; Group II experimental groupwere given0.5mg Penehyclidine intravenous at the beginning of intravenousanesthesia; Group II using intravenous anesthesia with small dose of saddleblock. The two groups in age, weight, surgery time, anesthesia timedifference not statistically significant (P <0.05).Record before the start of anesthesia as T0, after induction of anesthesiaas T1,after the start of surgery as T2, after the end of surgery as T3,respectively. Recorded at each time point the HR, HRv various indicators. Statistical analysis using the SPSS19.0software, the measurement dataexpressed as mean±standard deviation (±s), groups were analyzed usingindependent samples t-test, P <0.05significant difference statisticallysignificant.Results:High-frequency power (HF), low frequency power (LF), group I andgroup II compared with group I and group III, P <0.05; Compared withbefore anesthesia, T1and T0in Group II and Group III P <0.05; TP and LF/HF in Group I and Group III of the T2and T3time points, P <0.05.Conclusion:Penehyclidine for gynecological operative hysteroscopy is equivalent tointravenous anesthesia with small doses saddle anesthesia, lowerparasympathetic nervous excitement, inhibition of the vagus nerve effects, reflex sympathetic reduced volatility, stable internal environment, and a littleeffect on HRV. Facial nerve neurophysiological monitoring has no obviouseffect on anatomical retention rate of facial nerve after the operation ofacoustic neuroma.
