Objective:To investigate the clinical features of primary chronic daily headache andmedication overuse headache.Methods:Collected primary chronic daily headache and medication overuse headacheoutpatients in headache clinic of the Second Hospital of Jilin University from August2011to March2012, analyzed their general situation, headache characteristics,headache accompanied by symptoms and associated with symptoms and disabilitysituation, compared with the differences in headache characteristics, headacheaccompanied by symptoms and concomitant diseases of chronic migraine (CM) andchronic tension-type headache (CTTH), primary chronic daily headache accompaniedby medication overuse or not and medication overdose headache for different drugoverdose.Results:Collected a total of128patients with chronic daily headache, including26males,102female, female accounted for79.6%, with an average age of45.2years.88patients associated with drug overdose,40cases with PCDH are not associated withoverdose, which include18caseschronic migraine,17patients with chronic tensionheadache/possible chronic tension headache. Most chronic daily headache patients with previous long history of paroxysmal headache, and headache average duration of10years. PCDH, MOH accompanied by nausea, photophobia, phonophobia anddepression, incidence rate of irritability is more than50%, about half of the patientsaccompanied by fear olfactory, the phonophobia and emotional irritability appear inmore than70%of patients. PCDH, MOH associated with sleep disorders nearly75%,about50%with emotional irritability and dizziness, nearly20%associated withdepression change. MIDAS, HIT-6score of near2/3of PCDH and MOH showedsevere degree of disability.Overdose types of drugs of the MOH patients including non-steroidalanti-inflammatory drug (NSAID) and caffeine compound analgesics, Chinesemedicine, one-component non-steroidal anti-inflammatory drugs, the non-steroidalanti-inflammatory drugs and caffeine compound analgesics accounted for86%,compared with the clinical features of different drugs overdose group (generalsituation, headache characteristics and associated symptoms), the results showed thedifference is no significant. Compared with no associated with drug overdose PCDHpatients, the MOH patients have larger age, longer duration of total headache, severerheadache,and chronic migraine is the most common primary headache types.Conclusion:Most PCDH and MOH are middle-aged women, whose typical characteristics ofheadache degree of moderate to severe pain, they are more easily with depression,dysphoria and sleeplessness,could not enjoy working or life well and have highdisabling. Chronic daily headache patients are prone to drug overdose, drugs of overdose of the domestic patients are NSAID and caffeine compound analgesics.Weshould attach great importance to the regulatory medication to primary headache,prevent occurrence of headache chronic daily headache and medication overuseheadache, while paying attention to treatment of associated with symptoms.
