Part OneEfficacy and safety of patient-controlled sedation with transcutaneous electrical stimulation of auricular Shenmen point in cesarean sectionObjective To investigate the efficacy and safety of patient-controlled sedation with transcutaneous electrical stimulation of auricular Shenmen point in cesarean section.Methods A randomized controlled clinical trail was conducted on180singleton primiparas (SAS>30) undergoing elective cesarean section.The women were randomly divided into three groups:patient-controlled sedation with auricular transcutaneous electrical stimulation of auricular Shenmen point group(group A,n=60), patient-controlled sedation with auricular transcutaneous electrical stimulation of auricular Eye point group(group B,n=60) and control group(group C,n=60). Women of group A received patient-controlled sedation with transcutaneous electrical stimulation of auricular Shenmen point into the operating room(frequency1.5HZ).The strength was controlled by themselves. Stimulation for30minutes before the epidural puncture, continued to stimulate until the end of surgery; Women of group B received stimulation of auricular Eye point as group A; Women of group C received pressurization and connected line were the same with group A, but without electrical stimulation.Observation indexes:(1) the BIS, HR, MAP, Ramsay sedation score when the women came to the operating room (To),received30minutes stimulation (T1),remove the fetus (T2) and the end of surgery (T3);(2) the concentration of plasma Angiotensin â…¡ (Angâ…¡) and Cortisone(Cor) at the above time point;(3) the rate of oxytocin,atropine and ephedrine,the neonatal Apgar score.Results (1)Compared with group B and group C, the BIS, Ramsay sedation score,the concentration of Angâ…¡ and Cor were lower at the time of T1, T2, T3(P<0.05),HR and MAP were decreased at the T1time (P<0.05) in group A;(2)The rate of oxytocin, atropine and ephedrine,and the neor(?)al Apgar score were no difference (P>0.05).(3) The BIS, HR, MAP and Ramsay sedation score of group A were lower at the time of T1compare with T0(P<0.05),but group B and group C had no defference (P>0.05)Conclusion Patient-controlled sedation with transcutaneous electrical stimulation of auricular Shenmen point in cesarean section has obvious sedative effect,without adverse effects on maternal and neonatal. Stimulation for30minutes before the epidural puncture, continued to stimulate after the end of surgery for two hours; Women of group B received stimulation of auricular Eye point as group A; Women of group C received pressurization and connected line were the same with group A, but without electrical stimulation.Observation indexes:Observation timing:(1)from onset of anesthesia to removal of the fetus(T1);(2)from removal of the fetus to the uterine suture was completed(T2);(3)from abdominal exploration to the skin incision suture(T3);(4)from epidural morphine was given to after the end of surgery(T4) for two hours.Observation indexes:(1) the rate of nausea and vomiting, hypotension and lower heart rate;(2) the rate of metoclopramide,oxytocin,atropine and ephedrine;(3) the volume of blood loss and neonatal Apgar scoreResults Compared with group B and group C, the rate of nausea and vomiting were lower in group A (P<0.05) at the time of T3and T4, but no difference (P>0.05) at the time of T1and T2; the rate of metoclopramide was decreased in group A (P<0.05).The rate of oxytocin,atropine and ephedrine,the volume of blood loss and the neonatal Apgar score were no differences (P>0.05)Conclusion Patient-controlled transcutaneous electrical stimulation of auricular Shenmen point decreases the rate of nausea and vomiting from abdominal exploration to the skin incision suture and after epidural morphine was given in cesarean section,but has no effects from onset of anesthesia to removal of the fetus.
