| This thesis is divided into four parts.The first part is the general introduction of my mentor’s academic ideological originIt includes brief introduction of Professor Song Yiting’s life, academic background and pedigree of his successions from predecessors.Professor Song Yiting got high education in Traditional Chinese Medicine in his youth, and he has got lots of accumulation in TCM study through the extensive study and research as well as rich experience in clinical practices. In1979, Mr. Song got admitted as a postgraduate by the cardiovascular major combining TCM and western medicine, Shanghai College of Traditional Chinese Medicine. He had his knowledge come from Professor Wu Shishu, Professor Gao Zhaoji, and master in cardiovascular of western medicine Mr.Chen Haozhu. Mr. Song had made researches in the overlapping area of Chinese traditional medicine and western medicine such as coronary diseases, high blood pressure and geriatric rehabilitation medicine. Mr.Song employed modern scientific theories and measures to study Chinese Traditional Medicine, tried to find and explore the essence of the TCM from the perspective of modern medicine, testified the objective materiality of the their existence and completed a number of academic dissertations on Chinese and western medicine in research of coronary diseases.The second part is Summary and Conclusion of my mentor Professor Song Yiting’s academic thought.It gives a relatively comprehensive summary of my mentor through the past50years in clinical practices, during which time he has formed his own unique academic thought and clinical practices combining Western and Chinese elements.It mainly summarizes the academic ideas in the area of Obstruction of coronary heart disease treatment, put up the dialectical view that the pathogenesis of Obstruction of coronary heart disease is interconnectivity of five organs, holding the view that the pathogenesis of the coronary disease is connected with the five organs despite that its location of disease is in the heart area. The pathogenesis involves the changes of Yin and Yang in the blood and energy of the five organs, stems from the feebleness of the blood and energy, and has the syndrome of block of phlegm, blood stasis, obstruction of the energy, as well as Yang fever(over-energy). On treating the disease, clarification of deficiency and excess of the symptom have to be made clear, and treatment should be carried out in different stages, meanwhile, the whole situation of the disease should be taken into consideration so as to address both the symptoms and root causes, strengthening the body resistance to eliminate pathogenic factors, to nurture the body while treating in order to get a sound result.While making determination of etiologic factors based on differentiation, Mr. Song grasped the main aspect of the disease, taking the way of benefiting the vital energy, activating Yang, circulating blood, reducing phlegm as the fundamentals to treat Obstruction of coronary heart disease, and have the fundamentals played their roles through the whole treatment process. Meanwhile, combining the advanced examining methods and diagnosis in modern medicine, integrating the differentiating the disease with a dialectical manner, to further improve the effectiveness by uniting Chinese and western ways together and by suiting the remedy to the case.It mainly summarizes Hypertension diagnosis and treatment of academic thinking and Medicine of Mr. Song. Special attention to the balance of yin and yang of the liver and kidney, and Congenital deficiencies. That the incidence and pathogenesis of hypertension is kidney deficiency, when the gradual decline of blood and kidney failure, or due to long-term labor heart trouble, damage the heart yin, or the emotions of the fire, and other incentives to motivate, can lead to yin and yang balancedisorders, hyperactivity and hair of the disease.Treatment focus on nourishing the liver and kidney yin, Yuk Yin qianyang.Long-term imbalance of yin and yang, yin and yang deficiency can be caused by Vital Huoshuai, then need3-6grams the Guizhi or cinnamon to the Yin Yang yang fire, Return Fire element effect.This thesis makes a summary and overview on the academic thought established by my mentor in the treatment of cerebrovascular disease, by which the cerebropathy diseases are treated through various brain-smoothing and rejuvenating one’s vitality. He is good at employing the worm medicine, smoothing the brain, replenishing vital essence to benefit the marrow, to promote the blood circulation, eliminate the stasis and obstructions, Search carved collaterals stroke phlegm, tranquilize the liver, remove the endogenous wind, Antispasmodic faint, and promote the circulation of the energy and kill the pain. Meanwhile, Mr. Song put up his own points of view on the doze, type and side effects of the worm medicine.Mr. Song inherited and carried forward the academic thought of the preventive treatment of disease, particularly the application of preventive treatment of disease in treating the Heart cerebrovascular disease, which is of crucial importance to advance the disease-prevention and disease-treatment by TCM. Mr. Song attaches much attention on preventing disease from exacerbating, advocating the Rehabilitation care, which plays a very important part in the curing the Heart cerebrovascular disease such as stroke.By following my mentor in the treatment of clinical practices for three years, mass clinical experience has been accumulated. Summaries and overviews are made aiming at the treating of intractable insomnia, climacteric syndrome.depression and chronic heart failure associated with intractable edema.The third part is Literature Review of research development in treating coronary heart disease and angina pectoris by TCM.1.Overview of modern medicine on stable angina pectoris of coronary heart diseaseThis part mainly includes the introduction of west medicine concepts, clinical manifestations, check and diagnosis of stable angina pectoris of coronary heart disease.2.This part is the Overview of the research progress on Chinese medicine treatment of angina pectoris of coronary disease.This part includes the cognition of the disease of the ancient Chinesedoctors, modern research progress of the treatment of the angina pectoris of coronary disease by TCM, making a comprehensive summary about etiology and pathogenesis, syndrome classification, treatment of syndrome, acupuncture treatment and point application.The fourth part is also the core part of the thesis.This part includes researches on the prescription characteristic and therapeutic rules by Professor Song Yiting in the treatment of coronary heart diseases.1. The purpose of the studyIn this study, through the analysis and sorting of a large number of original clinical medical record information of my tutor’s daily treatment to coronary heart disease in the recent three years, fully tap the high frequency of occurrence of drugs and basic prescriptions in the medical cases; for commonly used medicines, explore the clinical treatment ideas and medicines prescriptions and regularity of my tutor according to the four properties and five flavors of the drugs, channel tropism and drugs efficiency analysis and sum up the theory, method, formula and prescriptions which reflects the core part of my tutor’s academic thought, promote prescriptions experiences for clinical practices and practicality in one which combines the practicality and practicability 2.The method of combining traditional clinical prescriptions copied from his predecessors and the collection and analysis of clinical cases by him is adopted.2.1Copying the prescriptions:Copying the different prescriptions from the master is a traditional way of learning which lasts for thousands of years, during which one can feel deeply and explore the ways of thinking and prescription features of one’s master.2.2Collection of the clinical cases and sorting work of the materials:by following the master in the clinical practices, collection of the clinical materials of treating the Obstruction of coronary heart disease by the mentor is made, including the analysis of the etiology and pathogenesis, the dialectical key points, ways of treatment and prescriptions, particularly, the specific elements, usage and dose of the prescriptions.2.3Selection standard of the prescriptions(1) All the prescriptions come from the patients with coronary disease receiving treatment by Professor Song Yiting from2008to2011.(2) The standards for diagnosis, inclusion and exclusionStandards for diagnosis: The standards for diagnosis on Angina Pectoris can refer to the diagnosis standards for ischemic heart disease by WHO; the standards for diagnosis of angina can refer to the diagnosis standards of ACC/AHA2002; the diagnosis standards for chest obstructions can refer to the syndrome classification standards of TCM in the guiding principles for clinical research in new TCM development.Standards for inclusion:all that is in line with the diagnosis standards for the western medicine in angina pectoris and in line with the diagnosis standards for the chest obstructions in TCM.Exclusion criteria:①combined with acute myocardial infarction②congestive heart failure, severe arrhythmia, severe pulmonary insufficiency, acute illness③integrating severe primary diseases of the liver, kidney, hematopoietic system.(3) Drugs name and dosage record is clear and complete on the prescription2.4. All the standard prescriptions are put into the computer through producing relative computer data forms and summarize and get the relative raw data to make statistical analysis of the data3. Dialectical type summary. 4. Input the relevant information through the production of computer data form, including all the prescriptions in accordance with the selection standards. A total of256standard prescriptions involving126kinds of medicines are selected out for statistical data analysis.Analyze the frequency and frequency rate of commonly used drugs of the first20kinds to make discussions over the prescriptions regularity of my tutor in his clinical practices based on the four properties and five flavors of the drugs, channel tropism and drugs efficiency analysis, and make back-extrapolation of my tutor’s prescriptions principles to sum up the theory, method, formula and prescriptions which reflects the core part of my tutor’s academic thought which will provide a theoretical basis for the future promotion and application in clinical practices.5. Conclusion:According to the usage frequency, drug properties and its effectiveness of the commonly used drugs by my tutor can back-extrapolate that his treatment ideas are characterized by smoothing tracheal pipes and activating blood circulation, promoting coronary circulation, dehumidification and reducing spitting, as well as eliminating stagnation in chest area when treating coronary diseases. Meanwhile, the academic thought on the pathogenesis of coronary diseases of my tutor can be further back-extrapolated and verified. With the guidance of the academic concepts and characteristics of long-term prescriptions of my tutor, the basic ingredients for the treatment of coronary diseases can be summarized as following:Salvia miltiorrhiza, Astragalus, heterophylla, Ligusticum wallichii, Cassia Twig, Pinellia, Trichosanthes, Longstamen Onion Bulb, licorice. These can lay a solid foundation for future research and development of new and specific drugs. |
