High Blood Pressure Early Renal Damage Survey And Syndrome Differentiation Of Syndromes Treatment Of 41 Cases Of Clinical Analysis With Early Renal Damage

Purpose:Recently there isn’t enough systematic research about TCM syndromes and Chinese medicine treatment on hypertension of early renal damage. This research through observing and statisticing the symptoms and indicators of patients with hypertension of early renal damage, to explore traditional Chinese medicine syndrome of this desease and the relevance with physical and chemical indicators. On the other hand, through a retrospective survey on the symptoms and indicators of patients before and after the Chinese medicine treatment, to explore the effect of Chinese medicine treatment of on this disease.Method:Using prospective study method by making clinical questionnaire and collecting data of114hypertension of early renal damage cases to study the TCM syndromes, related physical and chemical indexes. Though SPSS17.0with Clustering analysis method, and T tes、Variance analysis. A chi-square test、Pearson correlation analysis method to statistical analysis. Collecting data of41hypertension of early renal damage cases to study the TCM syndromes, related physical and chemical indexes before and after treatment, and though SPSS17.0with T test method to analysis Curative effection.Result:(1) Using Clustering analysis, to summary syndrome of traditional Chinese Medicine cases of114hypertension of early renal damage cases, can be divided into4types: Yin deficiency and yang hyperactivity syndrome, Syndrome of hyperactivity of liver Yang, Qi deficiency and phlegm stagnation syndrome, Deficiency of kidney and blood stasis syndrome. Yin deficiency and yang hyperactivity syndrome that accounts for the largest proportion,42.1%, Syndrome of hyperactivity of liver Yang is23.7%, Qi deficiency and phlegm stagnation syndrome is27.2%, Qi deficiency and phlegm stagnation syndrome is7.0%. Syndrome of blood stasis and heat-toxin syndrome are the main certification. The RBC and HGB Numerical value of Syndrome of hyperactivity of liver Yang is higher than Qi deficiency and phlegm stagnation syndrome, the BUN and K Numerical value of Yin deficiency and yang hyperactivity syndrome is lower than Qi deficiency and phlegm stagnation syndrome, Value of urinary occult blood of Syndrome of hyperactivity of liver Yang is higher than Qi deficiency and phlegm stagnation syndrome. Urine beta2-MG value of Qi deficiency and phlegm stagnation syndrome and Yin deficiency and yang hyperactivity is higher than Syndrome of hyperactivity of liver Yang, Circadian rhythm of blood pressure of Qi deficiency and phlegm stagnation syndrome is more significant change than others. Symptom scores of hyperactivity of liver Yang is realated to RBC. Neutrophils is significantly related to hemogolobin, urea, urea nitrogen level, and is negatively related to the RBC and hemoglobin, is positively related to neutrophils, urea, urea nitrogen.(2) Though an analysis of41cases of hypertension of early damage retrospective case study shows:Cobination of traditional Chinese and Western medicine treatment can reduce the dosage of medicine, has certain curative effect on the improvement of systoms and blood pressure control, unic acid, urinary MALB, urine P/C, urine NAG, urine beta2-MG and the number of nocturia reduce than before treatment.Conclusion:(1) The age composition ratio of hypertention of early damage is similar with the age composition ratio of hypertention. There is a large proportion of the patients who are overweight or with bad habits. The shortest history of hypertention is one month, and less than5years accounts for38.6%. This result is different with the common conclusion. The blood pressure is controlled not ideally, and the damage of heart and brain is still common. The main systoms are dizziness, dry mouth, acid, tibia and soft knee, irritability, insomnia, tinnitus, headache and chest abdominal stuffiness. The main tongue conditions are thin fur, white fur, dark tongue and yellow fur. The main pluse conditions are thready pluse and string-like pluse. TCM symptoms of hypertension of early renal damage Can be divided into4types:Yin deficiency and yang hyperactivity syndrome, Syndrome of hyperactivity of liver Yang, Qi deficiency and phlegm stagnation syndrome, Deficiency of kidney and blood stasis syndrome. Main ill position are Liver, kidney, heart, spleen. The main pathogenesis is yin deficiency of liver and kidney, phlegm and blood stasis.(2) Combination of Chinese traditional and Western medicine in treatment of hypertension of early renal damage, can better reduce the urine trace protein, improvement of renal filtration overweight absorpition function, effectively improve the clinical symptoms.
