| Cardiorenal Syndrome(CRS) refers to a pathophysiologic disorder of the heart and kidney whereby acute or chronic dysfunction of1organ may induce acute or chronic dysfunction of the other. With high incidence, complicated clinical symptoms, poor prognosis, and lack of effective treatment measures, CRS arouses the concern of cardiologists and nephrologists. The pathologic mechanism of CRS is very complicated. The current understanding of CRS in medical community is very limited. Some scholars regard that inflammatory reaction may be one of the main pathological mechanism, but the exact pathogenesis has not yet been elucidated. Clinically in the treatment of such patients, activating blood circulation herbs well protected myocardial ischemia, at the same time improve renal function, showing good prospects.The paper includes literature reviews and experimental research.Literature ReviewThe first review introduces the research progress of acute kidney injury from the following aspects, Definition, diagnostic criteria, diagnostic markers, epidemiological characteristics, treatment, prevention and prognosis.The first review expounds the cognition of acute kidney injury and the research progress of intervention of traditional Chinese medicine in patients with acute kidney injury.Experimental ResearchObjective:To observe rats’kidney damage after acute myocardial infarction and the expression of inflammatory cytokines in blood and kidney tissue, explore the possible mechanism of kidney damage after acute myocardial infarction inflammatory reaction. Observe intervention effect of xuefuzhuyu capsules in kidney damage of rats after acute myocardial infarction.Methods:Ligature the left anterior descending coronary artery of rats to create acute myocardial infarction models. The rats were divided into normal control group, sham operation group, the7days-model group, the7days-treatment group,14days-model group,14days-treatment group. Respectively test the ECG, biochemical index of blood and urinary, inflammatory cytokines in the blood and kidney tissue, and pathological changes of heart and kidney tissue, in the8th and15th day after operation.Result:1. The model rats’ level of ICAM-1, IL-1β, IL-10and TNF-a in serum, increased significantly (P<0.05).2. Both the expression level of ICAM-1mRNA and the activity level of NF-κB in renal tissue model rats significantly increased (P<0.05).3. Model rats occurred kidney damage:urinary NAG increased (P<0.05), renal pathological prompted hydropic degeneration of tubular epithelial cells;4. Xuefuzhuyu capsules decreased the expression level of inflammatory cytokines in blood and kidney tissue (P<0.05), and improved the pathological condition of model rats’renal tissue.Conclusion:Within14days, rats after acute myocardial infarction can occur kidney damage, and the damage is given priority to renal tubules; inflammatory reaction may be one of the main pathological mechanism of the kidney damage after acute myocardial infarction in rats; Xuefuzhuyu capsules has a protective effect on kidney of rats after acute myocardial infarction, the effect may also be related to the inhibition of expression of inflammatory cytokines. |
